How would you like to work in a high performing community mental health team serving rural Mid Bedfordshire, based in the idyllic surroundings of Steppingley Hospital?
We are looking for an enthusiastic and motivated Community Psychiatric Nurse to work as part of a multi-disciplinary working age community mental health team. You will be registered with the Nursing & Midwifery Council (NMC). We are keen to support a newly qualified nurse through their preceptorship and would offer a Band 5 position, but we would also welcome an experienced nurse to a Band 6 position.
The successful candidate will receive on-going support and supervision.
Your role will be to ensure the service provided by the team is well organised, evidence based, recovery focused, socially inclusive and person centred, with clear outcomes.

Main duties of the job
You will carry out the role of a care coordinator or named nurse under the principles of CPA. This will involve assessments, developing and implementing agreed care plans, reviewing the care provided, identifying risks and working with the patient to establish plans to mitigate risk, while responding to any emerging crises. You will be instrumental in providing psycho-education, mental state monitoring, relapse prevention work and offering advice and guidance on issues that may impact on someone's physical health. As a newly qualified nurse, you would be working under the direct supervision of our Senior Nurse.
You will be required to participate in the Duty rota and depot clinic rota, while being responsible for an identified caseload where you will need to ensure a high standard of professionalism and conduct which is consistent with best practice.
In the role, you will liaise with Primary Care services, the Voluntary and Private Sector and contribute to providing a Recovery focused model of care. You will play an active role in the discharge of our service users from hospital and will commission health and social care services as required.

1. Clinical Responsibilities
To attend the triage meetings to process referrals to determine priorities of needs and risks. To carry out mental health assessment including assessment of personal strengths and risks. To develop recovery plans using a person-centred approach to develop, implement and evaluate needs and strengths-based care plans and risk mitigation plans in partnership with service users, carers and other professionals. To liaise with professional e.g. GP at start of treatment, at review and discharge To offer health promotion and deliver harm reduction interventions and ensuring access to physical health care and health screening To provide one-to-one and group interventions. To provide evidence-based interventions in line with guidelines e.g. NICE, CQC in a variety of settings promoting flexibility and choice for service users. To deliver prescribing interventions, working in partnership with medical colleagues; monitor and review medication regimes and side effects To manage a caseload of service users, depending on complexity. To maximize engagement with service users and reduce the risks both to the individual service user and to others affected by their mental health symptoms. To facilitate service users and carers to be involved in care and treatment. To refer service users to appropriate Health and Social Care services that support their recovery and support them in accessing these services. To lead and participate in the clinical activities of the Team such as duty worker and depot clinic. To work as an effective member of the multi-disciplinary team ensuring effective communication with other team members and other professionals. To attend, lead and contribute to multi-disciplinary and multi-agency clinical meetings. To receive and participate in clinical supervision in accordance with Trust policy 2. Administrative

To undertake Trust mandatory training and appropriate training identified through supervision and appraisal. To identify own training, educational and development needs through appraisal and participate in a personal development plan to meet identified needs. To provide support, guidance and supervision to junior staff as appropriate. To provide mentorship to students/volunteers on placement. To participate in training within the Team and to other professionals and agencies. 6. Professional Responsibilities
