The role of a Care Coordinator includes coordinating care services and client schedules, matching carers with clients to meet individual needs, supporting the setup of new care packages, planning rotas to ensure continuity of care, communicating with families and healthcare professionals, recording staff availability, absences, and emergency calls, and participating in the on-call rota when required. We need someone who is organised and people-focused with strong organisational skills.

Qualifications Essential
Full UK driving licence and access to your own vehicle. Minimum of 12 months remaining on your right to work in the UK.
