Wyre Forest Health Partnership & WFNIPs is seeking an enthusiastic and experienced Financial Resilience Coordinator to join our Neighbourhood Outreach Team.
This is an exciting new role working across primary care, community services and the voluntary sector to help residents experiencing financial pressures, debt, deprivation, social isolation and the wider impacts of the cost-of-living crisis.
Working closely with Social Prescribers, GP practices, local authorities and community organisations, you will provide practical support that helps people stabilise immediate financial difficulties and build longer-term financial resilience. The role directly supports the ambitions of the Worcestershire Crisis and Resilience Fund, which focuses on crisis prevention, financial resilience and stronger community support networks.
You will provide direct casework, advocacy and practical support rather than simple signposting, helping individuals access the services and resources they need to improve their financial wellbeing. This reflects the identified need for hands-on support, warm handovers and ongoing case management.

About the Role
You will be embedded within our Neighbourhood Outreach Coordinator Team and work alongside social prescribing and community wellbeing services to identify and support residents who may be experiencing:
Financial hardship Debt concerns Benefit issues Fuel poverty Housing-related financial difficulties Social isolation linked to financial pressures Barriers to accessing advice and support

We operate at scale and are at the forefront of primary care delivery in how we organise ourselves, having management and leverage for a large number of primary care employees (c250 people & c50 GPs). We also look after c 72 000 patients. We have a very strong quality improvement ethic, and are always looking to improve.
WFHP is working towards the NHS goals to reach net zero greenhouse gas emissions as soon as possible. We actively encourage new staff to take an interest in this and build sustainable practice into their roles.

Key Responsibilities
Financial Resilience and Advice
Carry out holistic financial wellbeing assessments.
Complete welfare benefit and income maximisation checks.
Support residents with applications for benefits, grants and local support schemes.
Assist with evidence gathering, forms and appeals where appropriate.
Provide budgeting and financial capability support.
Identify opportunities to increase household income and reduce financial pressures.
Refer specialist debt, legal and housing matters to appropriate partners.
Neighbourhood Outreach
Deliver advice sessions within GP surgeries and community venues.
Accept referrals from GPs, practice teams, social prescribers and community partners.
Conduct outreach work across local neighbourhoods.
Undertake home visits where appropriate.
Provide responsive support to residents experiencing financial crisis.
Partnership Working
Build strong relationships with local VCSE organisations. Develop referral pathways with advice services and community groups. Work collaboratively with organisations such as Citizens Advice, Age UK, housing providers, family support services and energy advice providers. Attend neighbourhood MDT and partnership meetings. Promote available services and support opportunities across the local community.
Case Management
Maintain accurate and confidential client records. Monitor outcomes and impact. Produce reports demonstrating the difference made for local residents. Ensure compliance with safeguarding, information governance and data protection requirements.
Person Specification
Qualifications Essential
A-levels at grades A*C or equivalent
Experience Essential
Experience supporting people facing financial hardship. Knowledge of welfare benefits and income maximisation. Experience of working within advice, housing, health, social care or community sectors. Ability to manage a caseload and work independently. Excellent communication and relationship-building skills. Ability to explain complex information in simple, accessible language. Commitment to reducing inequalities and improving outcomes for vulnerable residents. Full driving licence and access to a vehicle.
Desirable
Experience within Citizens Advice, Age UK, housing associations or welfare rights services. Experience of social prescribing or community outreach. Knowledge of debt advice and financial capability approaches. Understanding of primary care and neighbourhood health models. Advice Quality Standard or similar advice-sector experience.
