Be part of a New Wave of Health for the Isle of Dogs
We are looking to recruit an enthusiastic and dynamic Clinical Pharmacist, to be part of a team working in primary care across the Isle of Dogs in Tower Hamlets.
The Healthy Island Partnership has been in operation for over 16 years and during this time has been at the forefront of many important changes to the health and wellbeing of local people. Excellent clinical achievement matched by work in the community around self-health has been a hallmark of Network success.
As a Clinical Pharmacist you will be responsible for working with and assisting practice teams in providing and supporting high quality clinical care for acute and chronic problems. Implementing effective medicine management; identifying areas for improvement and initiating and managing change.
This is a 30-hour, permanent position for someone with a Pharmacy degree and minimum 5 years post -qualification experience. The successful candidate must be willing to enrol on the CPPE Primary Care Pharmacy Education Pathway or submit evidence of exemption though past training and experience.

Main duties of the job
Be able to work flexibly and supportively in a general practice setting alongside other clinicians, particularly existing Clinical Pharmacist colleagues.Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.Help build the medicines management approach in designated practices across the Network, including delivery of training and advice and expertise to other Primary Care colleagues, to maximise cost-effective prescribing and improve the quality of patient care.To identify, develop and promote quality improvements in medicines management and pharmaceutical understanding across community pharmacy & GP practices ensuring that best practice is adopted.To develop and support new ways of working and workflow redesign linked to the safe, effective, high quality, use of medicines. by providing professional specialist medicines management.

Person Specification
Knowledge Essential
Independent prescriber.
Knowledge Essential
An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Qualifications Essential
Registered Pharmacist on GPhC Register.
Experience Essential
Minimum of 5 years post-qualification experience.
Skills and Abilities Essential
Able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
