Main duties of the job
We are looking for enthusiastic and proactive practitioners with strong initiative and a passion for contributing to the continuous development of our service. You will work in close partnership with Children and Family Hubs, schools, and other key agencies to ensure a holistic approach to care.
We have within Waltham Forest excellent opportunities within our 0-19 team for innovative and motivated Health Visitors who are keen to contribute to the further development of our service.

About us
Tower Hamlets GP Care Group (the Care Group) was incorporated as a community interest company limited by shares in September 2014. Every general practice in Tower Hamlets owns one share of £1. The Care Group was formed to manage borough level contracts on behalf of the practices, deliver primary care led services and support general practices, and to be the provider voice of primary care in the wider health and social care system.
The Care Group is also a member of Tower Hamlets Together, a place-based integrated partnership, which includes Bart's Health, East London NHS Foundation Trust, the London Borough of Tower Hamlets and the Council for Voluntary Services.

Job description Job responsibilities
Deliver the Healthy Child Programme, conducting mandated contacts and providing tailored support to families.
Work autonomously and as part of an integrated skill mix team, ensuring seamless service delivery.
Contribute to safeguarding children and young people, adhering to local and national guidelines.
Collaborate with multi agency professionals to address complex needs and improve outcomes.
Facilitate and support service development and change initiatives.
Effectively manage a caseload, including universal and targeted populations, ensuring equitable service provision.
Provide clinical supervision and mentorship to colleagues and students, fostering a supportive learning environment.
Maintain accurate and up-to-date records, adhering to data protection and confidentiality policies.
Participate in health promotion activities and community engagement initiatives.
Person Specification
Qualifications Essential
Specialist Community Public Health Nursing qualification (Health Visiting).
Skills and Abilities Essential
Demonstrate ability to manage and prioritise a caseload.
Experience Essential
Experience of specialist knowledge with reference to child development, perinatal mental health, child protection.
Skills and Abilities Essential
A range of clinical skills including assessment of development, health and identification of vulnerable children and their families.

Competent in the use of Information Technology.
