Hartlepool and Stockton Health are advertising for Dovecot Surgery.
We are seeking an experienced and motivated Nurse Practitioner to provide autonomous clinical care to patients across a broad range of acute and chronic conditions; a respiratory background would be preferred. This is an exciting opportunity to become part of a supportive and friendly team.

Job description Job responsibilities
Deliver high-quality clinical care, including assessment, diagnosis, and treatment of patients Diagnose and manage acute and long-term conditions. Independently prescribe medications within your scope of practice. Order, interpret and act upon diagnostic tests and investigations. Support long-term condition management and proactive patient care, providing health advice, disease prevention and lifestyle advice. Refer patients to secondary care and specialist services when required. Maintain accurate and timely patient records. Work collaboratively with GPs, practice nurses, ARRS staff, and the wider MDT Contribute to service development and quality improvement initiatives
Person Specification
Experience Essential
Strong clinical assessment and decision-making skills Ability to work autonomously and as part of a team Excellent communication and patient engagement skills A proactive and compassionate approach to patient care Ability to work autonomously and as part of a team.
Desirable
Experience managing chronic disease clinics. Lead on respiratory care, including COPD and asthma reviews, management, and optimisation in line with best practice Knowledge of UK primary care systems and pathways. Experience using SystmOne.
Qualifications Essential
Registered Nurse with current registration with relevant professional body. Qualified Nurse Practitioner with relevant prescribing qualification or working towards
