About this role

Job summary

We have an exciting opportunity to recruit 2 band 4 Cancer Care Coordinators to work alongside our Colorectal cancer specialist nursing teams providing non-clinical support. These exciting role will underpin the delivery of the personalised care agenda to people affected by colorectal cancer.

One post will be based in Darlington Hospital and one in Durham Hospital.

We are looking for highly motivated individuals who have excellent communication skills, are empathathetic, excellent problem solvers, and have a passion for delivering high quality person-centred care. You will provide the nursing team with administrative support, alongside non-clinical support and signposting for patients.

You will be qualified to at least NVQ Level 3 or have equivalent experience, possess good IT skills and have experience working in a healthcare setting. You will also be able to demonstrate resilience, compassion and a willingness to embrace innovation.

Main duties of the job

You will be based with the colorectal cancer specialist nursing team where you will assist with the day to day running of the service. This includes:

- Supporting the Multi Disciplinary Team meetings and outcomes to ensure the timely management of patients on a cancer pathway.

- Delivering personalised care through the use of Holistic Needs Assessments, health & wellbeing offers, Patient Statified Follow Up (PSFU), treatment summaries, referals to support services, and close working with the Macmillan Information and Support Team.

- Liaising with community and primary care teams to facilitate the timely management and treatment for patients.

- Advocating for patients, helping them navigate health & social care while empowering them to make the best choices for their needs.

- Providing support to those who may be upset or distressed.

- Considering any support needed in relation to health literacy and health inequalities.

- Dealing with enquiries from people dealing with cancer, these may be face to face, over the phone or by email.

- Visiting hospital departments to offer support to patients or family members as needed.

Some travel across sites is expected.

Job description Job responsibilities

You will work as part of the colorectal cancer specialist nursing team and the wider Cancer Services team to support the delivery of safe, compassionate and joined-up care. In this non-clinical role, you will work under the supervision of the Cancer Clinical Nurse Specialist team to assist in the provision of care. You may have some direct patient contact and your role will make a valuable contribution to improving the experience of people living with cancer.

You will contribute to Cancer Waiting Times (CWT) performance by supporting Clinical Nurse Specialists and patients throughout the pathway, helping to ensure appointments, investigations, and treatments are coordinated efficiently and delays are minimised.

You will be a flexible and adaptable advocate for patients, helping them to navigate health and social care while empowering them to make the best choices for their needs. You will possess excellent communication skills and will provide support to those who may be upset or distressed.

Working collaboratively with a wide range of services, you will ensure that people affected by cancer can access support in their local community, for example Citizens Advice, Joining The Dots, Social Prescribing Link Workers, Health Coaches and hospices.

Person Specification

Special Requirements Essential

Ability to concentrate on tasks whilst managing frequent interruptions. Ability to cope with exposure to details regarding patients that may be distressing. Ability and willingness to travel across trust sites as required. Professional appearance and demeanour. Positive attitude towards change. Flexible and adaptable. Highly motivated to learn new skills.

Qualifications Essential

GCSE English and Mathematics (Grade A - C) or equivalent qualification. NVQ level 3 in healthcare or admin subject, or equivalent workbased experience. Admin qualification or equivalent experience.

Desirable

Qualified to NVQ level 4 or above. Effective communication training/qualification.

Experience Essential

Previous administrative experience. Experience of using a wide variety of IT systems. Experience of working within the NHS in a care or admin role.

Desirable

Previous experience of working in the NHS in a role relating to cancer patient pathways. Experience of caring for with people with cancer.

Special Skills & Knowledge Essential

Ability to support a clinical team. Ability to work independently without direct supervision. An understanding of the importance of providing clear accessible information and support for people dealing with cancer. Understanding of person-centred care Awareness for the need for confidentiality at all times. Computer literacy ability to use a wide variety of NHS systems. Demonstrate the ability to solve problems and take appropriate action. Ability to organise and prioritise own workload. Ability to communicate effectively with sensitivity and compassion. Ability to work as part of the wider MDT. Willingness to undertake new skills appropriate to the service need.

Desirable

Knowledge of appropriate legislation with regards to sharing patient and staff data and protecting confidentiality. A basic knowledge of cancer and cancer treatments. Understanding of the Cancer Waiting Times targets and their application to the management of patient pathways. Experience of Audit Process.
