The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals the aim of reducing length of stay, preventing readmissions and optimising patient outcomes.
This is an exciting role which will require you to communicate effectively with patients, families and our multidisciplinary team in order to help patients get home as soon as they are ready. You will work with the teams on our wards at Abingdon Community Hospital. Your work will directly contribute to reducing length of stay, preventing readmissions, and improving patient outcomes.
Main duties of the job
Planning and Coordinating Hospital discharges Effective communication with patients and families to involve them in preparing for discharge from hospital Working closely with colleagues in other organisations such as adult social care and voluntary sector organisations to arrange appropriate support for patients when they leave hospital Maintaining clear and accurate documentation Coordinating and contributing to daily board rounds and weekly MDT meetings. Contribute to the induction and training of new staff, including shadowing and mentoring opportunities relevant to the discharge process. If you enjoy working in a fast paced role and solving problems then this could be the ideal job for you as no two days are the same for the patient flow team. Provide weekend cover as needed and cover other Community Hospitals when required.
