About this role
SUMMARY OF DUTIES Responsible for rendering professional nursing care to patients within an assigned unit of the hospital. Such nursing care is an integral part of the individualized holistic patient care plan as directed by the Medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.
MAIN DUTIES RESPONSIBILITIES 1.Understands and carries out the nursing care in line with Vision and Mission Statements, Goals and Objectives that suit the scope of the Nursing Services. 2.Works shift duties or as assigned by Unit Manager according to Unit’s Operational Hours. 3.Informs Unit Manager when change occurs in circumstances which affect standards of care. 4.Receives and handover report of patient, knows the case in detail:
• Progress of the patient. • Ensures all doctors’ orders are carried out before shift change and on taking over case. • Any incident or problems encountered. 5.Visits every patient in the unit together with the team members on taking over of duties to:
• Assess condition of patient, compare with report. • Re-plan nursing care according to assessment or evaluation • Determines nursing priorities and plans patient care according to patient’s needs and staff availability.
6.Provides individualized holistic care to patients using the Nursing Process approach with proper nursing documentation in patients files and that it is contemporaneous. 7.Reports all abnormal findings and progress of patients to the relevant doctors / consultants. 8.Conducts regular nursing rounds on all patients in the ward to ascertain for other additional needs; talk to patient/family members as an integral part of providing emotional support. 9.Follows doctors / consultants ward round and assists them in doing procedures when required. 10.Carries out orders of doctors / consultants pertaining to care of patients and delegates to other team members according to their assignments. 11.Liaises and discusses with the relevant doctors / consultants with regards to care of patients in the unit / ward. 12.Establishes and develops effective communication with all disciplines, patients and relatives, to achieve optimal patient’ satisfaction. 13.Attends and participates in unit meetings, dialogue sessions and hospital’s activities. 14.Participates in the health education of patients and their families in the unit. 15.Assists the Unit Manager in the administration of the unit / wards when assigned as shift nurse in charge. 16.Acts on behalf of the Unit Manager in setting standards and expectations in the quality of care. 17.Carries out duties and responsibilities of the Unit Manager in her absence as directed 18.Ensures charges of equipment/ consumable items into patient’s charges accordingly and checked for completeness. 19.Responsible for checking and documenting supply of sterile packs from CSSS and weekly checking for integrity of packaging 20.Maintains and documents adequate supply of unit stock. 21. Carries out assignments scheduled by Unit Manager and provides feedback
ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREAS
Staff nurse in Hemodialysis 1.Capable in performing principles, methods and skills in providing high standard of nursing care to patients on Hemodialysis machine, Continuous Renal Replacement Therapy (CRRT), Renal Replacement Therapy (RRT) Internal Jugular Catheter (IJC), Femoral Catheter and, Arterio-venous fistula line and Arterio-venous graft. 2.Capable in performing of principles, methods and skills in providing high standard of nursing practices in care of pre, intra and post hemodialysis treatment. 3.Assist the Unit Manager in clinical guidance and supervision of nursing personnel in activities directed toward maintaining standards of patient care, safety, infection control and professional behavior. 4.Ensure the implementation of infection control and safety measures concerning all patients in the unit, the storage and administration of bloods and drugs, radiation, static electricity, fire and unit cleaning. 5.Participate in giving support and health education to dialysis patient especially first time patient to care for themselves at home. 6.Participate in advising patient for involving in organ donation programme. 7.Participate in planning patient’s treatment programme to achieve adequacy of treatment
QUALIFICATIONS REQUIRED Certificate/Diploma in Nursing/Bachelor of Nursing Post Basic in related field - Renal Care Nursing.
EXPERIENCE REQUIRED 1.Registered with Nursing Board of Malaysia with a valid license to practice as a Registered Nurse. 2.Registered Nurse with 1 year experience 3.Able to write and communicate fluently in Bahasa Malaysia and English language. 4.Strong interpersonal and communication skills. 5.Willingness to work on scheduled hours, shift, and weekends, public holidays and anytime when called during emergency. 6.Microsoft office literacy
