Main duties of the job
Lead chronic disease management clinics for asthma, COPD and hypertension. Conduct comprehensive health assessments, medication reviews, and lifestyle counselling. Provide health promotion, vaccination services, and preventative care screening. Perform clinical observations, wound care, and minor procedures. Support clinical governance, safeguarding, and infection control protocols. Collaborate with GPs, Practice Nurses, and wider healthcare teams. Mentor healthcare assistants and support practice development initiatives. Maintain accurate clinical records and contribute to quality improvement projects

Key Clinical Responsibilities
Chronic Disease Management
Lead chronic disease clinics for asthma, COPD, hypertension, and other long-term conditions
Conduct comprehensive health assessments and annual reviews in line with national and local guidelines
Medication management including reviews, reconciliation, and monitoring
Patient education and self-management support for chronic conditions
Care planning and goal setting with patients and families
Health Promotion and Prevention
NHS Health Checks and cardiovascular risk assessments
Health promotion activities including smoking cessation, weight management, and lifestyle advice
Vaccination services including seasonal flu, COVID-19, and travel immunizations
Screening programs including cervical screening.
Preventative care and early intervention strategies
Clinical Assessment and Treatment
Clinical procedures as trained and assessed competence Comprehensive nursing assessments using systematic approaches
Clinical observations including vital signs, ECGs, and physiological monitoring
Wound care management including assessment, treatment, and healing monitoring
Minor illness management within scope of practice and competency
Clinical procedures as trained and assessed competent
Multi-disciplinary team working with GPs, Practice Nurses, and allied health professionals
Care coordination and case management for complex patients
Referral management to secondary care and community services
Liaison with district nurses, social services, and external agencies
This role requires flexibility to work across different clinical areas within the practice, adapting to varying patient needs and service demands. Person Specification
Experience Essential
Experience working in healthcare setting prior to foundation degree training Post-registration experience as a Nursing Associate Experience in patient assessment and care planning
Desirable
Experience in Primary care or community healthcare Experience using System one Chronic disease management and health promotion.
Qualifications Essential
- NMC registration as a Nursing Associate Foundation Degree in Health and Social Care or equivalent Current NMC PIN and professional indemnity - Qualified in Phlebotomy
Desirable
- Wound care knowledge including compression bandaging and Doppler - Qualified in Long term conditions e.g. Asthma & Diabetes - NHS Health Checks trained.
