Job summary <p>Are you an experienced Physiotherapist looking for an opportunity to help shape a brand new service and make a lasting difference to people across Medway?HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service from 1 October. As we launch this exciting new service, we are looking for a Specialist Physiotherapist to join our growing team and play a key role in supporting people to recover, regain independence and achieve the outcomes that matter most to them.This is an exciting opportunity to join a service at the beginning of its journey. Working alongside colleagues across health and social care, you will help develop and deliver high-quality rehabilitation and reablement pathways that support a Home First approach, improve patient outcomes and enable people to live as independently as possible within their own homes and communities.</p><h4><br></h4><h4>Package Description</h4><p>You will be joining a passionate and supportive multidisciplinary team dedicated to helping people maximise their independence following illness, injury or hospital admission.
