Job summary <p>As a Band 5 Community Nurse, you will join our valued Community Nursing Team based at Sheppey Community Hospital, providing care across Sheppey, Sittingbourne and the surrounding villages. This is a rewarding opportunity to deliver high quality nursing care where it matters most, within patients\' homes and local communities. You will <span class="single_desc_int_gb_word">support patients with a wide range of healthcare needs, helping them maintain independence, manage long-term conditions and avoid unnecessary hospital admissions</span>.
