Job summary
This role will be responsible for supporting practices across the Primary Care Network (PCN) and will work in locations within the community to provide clinical services. The areas covered by the PCN are Holt Park, Horsforth, Burley, Kirkstall, Headingley, Woodhouse, Hyde Park.
Main duties of the job
The Social Prescribing model aims to improve physical, mental and social health and wellbeing, through a holistic approach to improving quality of life and confidence in self-management and supporting independence. Working with our local population to improve the way the needs of local populations are met. This will be both on a 1:1 basis and where appropriate through the development of group work.
The team take referrals to support individuals to build independence and personal resilience, enabling self management of their health and wellbeing tailored to individual need. They will also take a lead in supporting administrative staff to develop their signposting skills raising awareness of the different services on offer locally.
The post holder will lead a team of 6 social prescribers and work closely with GP practices, the PCN Leadership Team and the PCN Manager.

Woodsley & Holt Park PCN is a large primary care network (PCN) with a combined patient population of approximately 100,000 across 10 GP practices, covering the areas of Woodhouse, Kirkstall, Burley,Hyde Park, Headingley,Cookridge and Horsforth. . The PCN footprint is of mixed diversity with pockets of deprivation and health inequalities. As one of the largest PCNs in Leeds we work at pace to deliver high quality services to our patients, which will evolve with the continuing development of the PCN.

LEADERSHIP AND MANAGEMENT TASKS
As the lead Social Prescriber you will be expected to contribute to the leadership for the PCN, and to provide leadership and mentoring to any of the social prescribers, employed across the PCN.
Demonstrate understanding of the social prescriber role and is able to implement this appropriately within the workplace.

OTHER TASKS
To promote the service within GP practices and develop close working relationships with the teams within practices and the Primary Care Network.
To complete comprehensive and holistic assessments of need for those referred to the service within agreed timeframes.
To triage referrals and signpost to specialist support quickly where necessary
To support administrative staff to develop their signposting skills, to ensure patients are directed to the most appropriate service
To work with a caseload of individuals to facilitate access to recommended community-based interventions and programmes of activity that benefit health and wellbeing. To support individuals into provision within agreed timeframes
To develop collaborative Wellbeing support plans with individuals which promote choice and complement any medical treatment or advice.
To identify any barriers to people accessing services and activities and work with individuals to overcome these.
To use a range of techniques and interventions to support people, including those with long term conditions, to self-manage their health and encourage behavioural and lifestyle change.
To monitor and record contacts and outcomes within SystmOne & Emis
To build networks with local community resources such as activities and services that may have an impact on health and wellbeing and support individuals to access these
To develop multi-agency working across communities and voluntary and statutory organisations to improve health and address health inequality

Experience Essential
At least 3+ years of experience working directly as a Social Prescribing Link Worker, health advisor, or community development worker. Proven experience managing or supervising staff/volunteers, running appraisals, or coordinating caseload allocations. A track record of working closely with multi-disciplinary clinical teams, such as General Practitioners (GPs), nurses, or social workers
Qualifications Essential
GCSE English and Mathematics at Grade C / 4 or above (or equivalent) NVQ Level 3 or 4 (or equivalent) in Health and Social Care, Public Health, Community Work, or a related discipline.
Desirable
Leadership or management qualification (e.g., ILM Level 3/5, CMI, or equivalent management training). Formal training in Safeguarding
