About this role

Job summary You'll support people receiving rehabilitation care, helping them maintain their wellbeing, independence and quality of life. Working as part of a caring and supportive team, you'll help deliver person-centred care that focuses on what matters most to each individual. Your contribution will make a real difference to patients' experiences by providing practical support, encouragement and compassionate care every day.

Main duties of the job What you'll do:

Support patients with care activities in line with their individual care plans Carry out agreed clinical tasks, such as observations, blood pressure monitoring and specimen collection following training and competency assessment Communicate effectively with patients, carers and healthcare professionals Record patient information accurately on electronic systems and support basic administrative tasks Help maintain a safe, organised environment, including preparing equipment and managing stock levels What we're looking for: A Level 2 qualification in Adult Health and Social Care, or equivalent experience Experience supporting people in a healthcare, homecare or care home setting Strong communication and interpersonal skills, with a caring and respectful approach Ability to work independently and as part of a team Willingness to complete the Care Certificate during induction if not already achieved We value people from all backgrounds and experiences and are committed to creating an inclusive workplace where everyone is treated with fairness, dignity and respect. Person Specification

Qualifications Essential

Level 2 qualification or equivalent level of experience in Adult Health and Social Care Hold Care Certificate or willingness to complete within induction period.

Knowledge Essential

Knowledge of working in a health care environment

Desirable

Good understanding of basic IT skills

Experience Essential

Recent experience of working with patients in a healthcare, homecare or care home setting Experience of prioritising own workload
