About this role

Job summary An exciting opportunity has arisen to join the North Community nursing team as a clinical practice Teacher. The post holder will be responsible for day-to-day clinical management of the team and will liaise and work with the Clinical Team Leader to ensure an integrated and organised approach to safe effective patient care delivery and operational service delivery.

Main duties of the job What you'll do:

Plan and deliver practice education for pre-registration and post-registration learners Assess patients with complex or long-term needs and develop evidence-based care plans Manage a community caseload, prioritising care and delegating work safely Lead clinical supervision and support colleagues to build skills and confidence Work with health, care and university partners to improve learning and patient services Some experience of direct line management, supervision and HR policies and procedures would be advantageous. What we're looking for: Registered General Nurse with a relevant community nursing teaching or practice educator qualification Practice Assessor or mentorship qualification and Community Nurse Prescriber V150, or willingness to work towards it Experience of practice education, clinical leadership, team supervision and caseload management Clear communication, coaching and organisational skills Confidence using electronic patient records and digital learning tools We welcome applications from different backgrounds and communities. Qualifications Essential

RGN/RN/Diploma Nursing Studies Non-medical prescribing V100/V150 or willing to work towards within specified timeframe Specialist training/short courses relevant to the post or equivalent level of experience

Desirable

Specialist Community District Nursing post graduate diploma

Knowledge Essential

Current Legislation and impact on community nursing services. Evidence based practice Mentorship or evidence of supervision/teaching/team management Evidence of understanding of the role of Case Manager and care coordination. To demonstrate an understanding of proactive caseload management Integrated Team working

Desirable

Understanding of audit processes, Key Performance Indicators and Targets, CQUIN collection data and Quality Monitoring X P286 Case Manager Evaluated 26th May 2016 8 Programmes Integrated Team working Understanding of research methodology and audit To have an understanding of the principles of restorative practice
