Are you a passionate and motivated Registered Nurse looking to make a real difference to patient outcomes? This is an exciting opportunity to join ournew Home First service that launched on 1 September, supporting patients to return home safely from hospital and avoid unnecessary admissions.
Working as part of a multidisciplinary team, you will deliver timely, holistic assessments and develop personalised care plans for patients with complex health needs. You will play a key role in supporting hospital discharge, promoting independence, and ensuring patients receive the right care, at the right time, in the right place.

Main duties of the job
Complete holistic nursing assessments and develop personalised care plans for patients discharged through Home First and other discharge-to-assess pathways. Support safe and timely hospital discharges and help prevent avoidable hospital admissions. Deliver evidence-based nursing care, treatment, and intervention for patients with complex, acute, sub-acute, and long-term conditions. Identify ongoing nursing needs and coordinate seamless handover to Community Nursing Teams where required. Work collaboratively with therapists, reablement staff, GPs, social care teams, and other system partners across One Gloucestershire. Promote independence through health coaching, education, goal-setting, and personalised care approaches. Support medicines management and clinical decision-making to optimise patient outcomes. Provide advice and support to patients, carers, and families to maximise health, wellbeing, and self-management. Contribute to service improvement, quality assurance, audit activity, and patient safety initiatives. Support the development of colleagues and students through supervision, coaching, and leadership within the team. Manage and prioritise a dynamic caseload, demonstrating excellent clinical judgement and organisational skills.

QUALIFICATIONS Essential
Registered Nurse (Adult / RGN) Current NMC registration Holds degree in health related subject or be working towards this, with over 50% of level 3 or 6 credit already obtained Individual modules at degree or masters level, eg. PACR Holds recognised teaching/ mentoring qualification
LENGTH AND / OR NATURE OF EXPERIENCE Essential
Experience of working without direct supervision Consolidated experience of community nursing practice Experience of managing long term conditions and case management of complex patients Experience of delivering extended role clinical interventions Evidence of post-registration education & training.
PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE Essential
