Job Description
SWAGGA Primary Care Network (PCN) is a dynamic collaboration between fifteen practices across South Liverpool. We have a vision to improve patient care, tackle inequalities and provide outstanding support for our member practices. We plan to make a lasting impact upon the communities we serve and are developing ambitious plans for the future. As a result, we are looking for qualified GPs, to deliver a hybrid model of Hub based face to face clinics and home visits to support our practices. The post-holders main duty will be to conduct face to face appointments within a hub for our practices and home visits to undertake consultations with patients. The home visits will be face to face appointments, visiting patients in their own homes, triaged by the patients practice GP.

About us
SWAGGA Primary Care Network is a collaboration of 15 GP Practices geographically aligned and working together to improve patient care. SWAGGA PCN has a weighted patient population of approximately 100,000 patients. It is an extensive Network with a diverse population and patient needs.
Our Network Team work across these practices, aiming to improve our patient's well-being and address health inequalities in South Liverpool.

As a result, we are looking for qualified GPs, to deliver a hybrid model of Hub based face to face clinics and home visits to support our practices. The post-holders main duty will be to conduct face to face appointments within a hub for our practices and home visits to undertake consultations with patients. The home visits will be face to face appointments, visiting patients in their own homes, triaged by the patients practice GP. To comply with ARRS funding criteria, the role will be embedded within the PCNs Core Network Practices and be fully integrated within the multi-disciplinary team delivering healthcare services to patients. It will also have access to other healthcare professionals, electronic live record systems of the PCNs Core Network Practices, as well as access to admin/office support and training and development as appropriate. It will also have access to appropriate clinical supervision and administrative support.
NB. There will be no Care Home or End of Life appointments as part of this role.

We are looking for a passionate and dedicated qualified GP to join SWAGGA PCN. This is an exciting opportunity for a GP who has completed their CCT (Certificate of Completion of Training) and is seeking a dynamic and supportive work environment. As an ARRS funded qualified GP, you must not currently be employed in a substantive post within the SWAGGA PCN network, and you will be dedicated to providing high-quality patient care.

Our adjusted patient population is approximately 100,000 and we work very closely with our community team and other local Healthcare providers. This includes Salaried GP's, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Social Prescribers and First Contact Physios.
