About this role

Job summary

We are looking for a Care Navigator to join our friendly practice on a part-time basis.

Hours: 19 per week

Monday 08.00 - 14.00Wednesday 14.00 - 17.00Thursday 14.00 - 18.45Friday 13.30 - 18.45We're open to patients from 08.00 to 18.30, Monday to Friday, and shifts are scheduled within these times. From time to time, you'll also be required to work a late shift finishing at 8pm.

The hours listed in this advert are the exact hours we need to cover, so please only apply if you can commit to these as stated. We cannot offer flexibility around start or finish times.

Main duties of the job

You will be the first point of contact for patients and other contacts, you will help patients navigate our patient triage system, book appointments, deal with general enquiries, and support the day to day running of the team.

We're a friendly and well-established GP practice based in the beautiful cathedral city of Wells, Somerset England's smallest city, rich in history and community spirit. We care for a patient population of around 8,500 and are proud to be part of the West Mendip Primary Care Network.

Our clinical team includes six GP partners, two salaried GPs, three practice nurses, and three HCAs, supported by a visiting paramedic and a welcoming reception and admin team who help keep everything running smoothly. We work closely with other local practices in our PCN to provide joined-up care and to make the most of shared resources and specialist services.

Job description Job responsibilities

We're looking for a Care Navigator to join our friendly reception and admin team. This is a key front line role where you'll be helping patients get the care they need, while supporting the wider practice with a range of admin and coordination tasks.

Your day-to-day work will include greeting patients, booking appointments, answering queries (in person and by phone), and signposting patients to the right service or clinician. You'll also process home visit requests, handle incoming mail, update patient records, and liaise with healthcare professionals both in and outside the practice.

You'll need to be confident using computers and able to stay calm and organised in a busy environment. Good communication skills are essential, along with the ability to use your judgement and initiative especially when helping patients who need urgent attention.

Other duties include helping keep the reception area tidy and welcoming, supporting with opening and closing the building, and assisting with general practice tasks as needed

As a busy GP practice, we value a team approach and would welcome candidates who can offer a reasonable degree of flexibility to help cover annual leave and occasional sickness absence when required.

Experience in a similar role is an advantage, but full training will be given. Most importantly, were looking for someone reliable, adaptable, and committed to providing a great service to our patients.

- GCSE or equivalent including maths and English
