About this role

Job summary

This is Band 6 post has been newly created to provide increased capacity for assessments of new referrals to the team, at a time of increasing demand. The successful applicant will have the opportunity to work with service users presenting with a variety of mental health conditions including organic and functional. As part of an experienced, skilled and supportive multi-disciplinary team the post holder will be acting as Key Worker to deliver evidence based interventions to service users and support to carers.

Main duties of the job

Main duties of the role:

To facilitate the delivery of personalised care and support planning in line with 'Your Team, Your Conversation, Your Plan.

To provide personalised interventions to support people in Later Life with mental health difficulties.

To act as a Key Worker for people for whom you are delivering interventions.

To deliver a range of activities and interventions to support carers.

To provide mentoring and training for others in relevant practice areas.

Other requirements Essential

Mobile with the facility to move quickly across a geographically dispersed area where there is limited access to public transport.

Education and Qualification Essential

Diploma level/ Degree in Nursing Current registration with the NMC

Desirable

Relevant post -registration qualifications and experience

Skills and abilities Essential

Substantial post-registration experience working in adult mental health

Desirable

Experience gained in a variety of settings including people in Later Life
