We are looking for a proactive and experienced QOF care coordinator to join our friendly and well-established GP practice team. Working across within our family-oriented Surgery. You will play a key role in supporting the delivery of high-quality patient care through the effective management of Quality and Outcomes Framework (QOF) targets, enhanced services, patient recalls, and data quality initiatives.

The QOF & coordinator will be responsible for supporting the delivery and achievement of Quality and Outcomes Framework (QOF) targets, enhanced services, and wider practice performance objectives within Lockswood Surgery.
The post holder will undertake patient recalls, run clinical searches and reports, monitor performance against targets, support data quality and clinical coding activities, and work closely with clinicians and administrative staff to improve patient outcomes. The role will also include general administrative duties, maintaining accurate records, producing management reports, and ensuring compliance with practice policies.

The QOF care coordinator will support the delivery of high-quality patient care by coordinating Quality and Outcomes Framework QOF activities, enhanced services, patient recall programmes, and data quality initiatives across the practice. The post holder will work closely with clinicians and administrative staff to ensure that contractual targets are met, patient records are accurately maintained, and performance is monitored effectively.
QOF and Performance Management
Monitor and support the achievement of QOF targets and other contractual requirements.
Run regular clinical searches and reports to identify patients requiring review, monitoring, or intervention.
Maintain and manage patient recall systems for chronic disease management, screening programmes, immunisations, and health checks.
Support clinicians in identifying care gaps and opportunities to improve patient outcomes.
Produce performance reports and information for meetings.
Person Specification
Qualifications Essential
Essential Experience working within a GP practice. Understanding of QOF and patient recall processes. Experience using EMIS Web Excellent organisational and administrative skills. Strong IT skills, including Microsoft Office applications. Ability to prioritise workload and meet deadlines. Excellent communication and interpersonal skills. Understanding of confidentiality and data protection requirements.
Desirable
Knowledge of enhanced services and NHS contracts. Experience of clinical coding and data quality improvement. Experience producing reports and conducting audits.
