About this role

Job summary

An opportunity has arisen for an enthusiastic, well organised, experienced nurse to join our friendly and supportive team to provide a high standard of service to our local population.

We perform well, both in terms of Bolton Quality Contract and QOF outcomes, and provide a wide range of enhanced services.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available.

Main duties of the job

Conduct patient health reviews for Long Term Conditions, development of individual patient centred management plans at annual review; agreeing these as appropriate with the patient and other health professionals in accordance with current guidelines and with knowledge of QOF requirements and NICE guidelines Performing medication reviews and assessments for long term conditions Educate, advise and support newly diagnosed and established chronic disease management patients NHS Health Checks/New Patient Checks Assist at IUD fitting clinics Testing Urine samples Blood results reporting (training and protocols provided) Assessment and administration of travel vaccinations B12 injections Depo administration ECGs Smears Pill checks/HRT checks Ordering & Administering appropriate immunisation and vaccinations Communicating with Child Health and D of H on immunisation queries and programmes

Maintenance of cold chain and fridge temperatures Collaboration of equipment

About us

We are a training practice consisting of 7 GP partners, 2 Salaried GPs, 2 ANPs, GP Trainees, Paramedics, Pharmacists and Practice nurses. Our list size is approximately 11,000.

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. Maintain accurate records following NMC standards of record keeping. Contribute to the practice achieving GMC contract obligations.

The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available.

Person Specification

Qualifications Essential

Nursing Registration with the NMC Educated to degree level or equivalent

Desirable

Non medical Independent Prescriber Qualified in Phlebotomy

Good interpersonal and communication skills. IT skills. Role-models the values, behaviours and interactions expected and ensures patient, family and carer feedback support this in practice. Make professionally autonomous decisions and provide safe evidence, based cost effective, individualised patient care.
