We are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Northbrook, IL. area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
What You’ll Do:
• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life • Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders • Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care • Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation • Initiate evaluations to other disciplines when indicated • Work independently, but never alone • Travel to patient homes within assigned territory

Admission RN Qualifications
• Active Registered Nurse, RN license in the state you work • Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred • Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
