An exciting opportunity has become available for a skilled, passionate Band 6 Occupational Therapist to join our forward-thinking Care Home Intensive Support Team (CHIST). This is a rewarding, fast-paced community role where your expertise will directly enhance the health, wellbeing and quality of life of care home residents across the county.
Working autonomously with remote working flexibility, you'll travel to care homes registered with Staffordshire County Council and Stoke on Trent City Council, delivering targeted, impactful interventions exactly where they are needed most.
About the Role
CHIST delivers time-limited, focused Nursing and Occupational Therapy and Speech and Language Therapy support to care homes identified through local system escalation processes. As part of this innovative service, you'll be at the heart of improving outcomes, reducing risk and supporting care home teams to deliver consistently high-quality care.
You will quickly build strong, collaborative relationships with care home staff, using your excellent communication and interpersonal skills to embed best practice and confidence within teams.
Main duties of the job
What You'll Be Doing
Providing hands-on specialist Occupational Therapy support tailored to resident needs
Offering expert advice and guidance on areas such as:
Moving and handling
Falls prevention and management
Promoting meaningful activity and engagement
Personalised care planning
Nutrition advice and appropriate signposting
Navigating and strengthening links with health and social care services
Supporting the day-to-day organisation and operation of the team, working closely with team leaders
Demonstrating strong clinical leadership and risk management skills, prioritising care home support and escalating concerns appropriately
Promoting effective teamwork within care homes and sharing knowledge, skills and experience in line with evidence-based practice

Person Specification
Qualifications & Training Essential
Degree in occupational therapy or equivalent Registration with HCPC Mentorship qualification (998-997) CPD Folder / Evidence of ongoing learning/ development Post qualification courses in occupational therapy Attendance at relevant networks e.g. Special Interest Groups Willingness to take students and complete student education training Demonstrate understanding of Equality and Anti discriminatory practice
Desirable
Modules at Masters level Post Graduate studies in relevant occupational therapy field such as; rehabilitation, end of life care, long term conditions/palliative care or related/relevant subject Evidence of further clinical study Up to date field work/pre-registration student training
Experience Essential
Relevant experience to the role including knowledge and management of long term conditions Ability to manage time and workload effectively Working in a multidisciplinary team To have knowledge & experience working with older adults
Desirable
To be able to negotiate and manage conflict in clinical situations To be able support in service improvement To have knowledge & experience working in both nursing & residential care settings
