About this role

Job summary

We are looking for a pharmacist to join our team to support medicines management and improve our patient care. The ideal person will have Independent Prescriber status or be willing to undertake this training if not yet qualified.

The role will be 18 - 22 hours and the hours of work are flexible during the week so ideal to fit around other roles, childcare or outside commitments.

Main duties of the job

Working with the GPs, practice nurses and practice staff to:

Improve the quality and effectiveness of medicines management in the practice

Provide patient focussed, prudent healthcare

Promote medicines safety

Work as an integral member of the team to improve the quality, safety and effectiveness of prescribing through the development of robust systems and processes

Job description Job responsibilities

Key Responsibilities are

Clinical Patient Care

Prescribing and Medicines Optimisation

Review and improve prescribing practice in line with local and national guidance

Develop and embed safe, efficient prescribing processes within the practice

Support cost effective prescribing while maintaining quality and patient safety

Monitor and action prescribing data, audits, and quality improvement initiatives

Contribute to practice achievement of contractual targets through the planning, co ordination and delivery of prescribing based projects and audits

Support safer prescribing of high risk medications e.g. anticoagulants, opioids, DMARDs

Process Development and Quality Improvement

Design, implement, and refine protocols, SOPs, and medicines workflows

Lead or contribute to clinical audits and service improvement projects

Promote safe repeat prescribing systems and medicines monitoring processes

Multidisciplinary Working

Work collaboratively with the GP and nursing team and other allied health professionals

Act as a medicines expert within the practice

Liaise with community pharmacies and secondary care as appropriate

Support training and medicines education for practice staff

Professional and Governance Responsibilities

Practice in accordance with GPhC standards, clinical governance, and local policies

Maintain accurate and timely clinical records

Engage in continuing professional development CP, and clinical supervision

Participate in appraisal, reflective practice, and service development planning

Person Specification

Experience Essential

Experience working in a patient facing clinical role Excellent communication and consultation skills Ability to work autonomously and as part of an MDT Commitment to patient centred care and quality improvement

Desirable

Previous experience in general practice or primary care Experience of developing or improving prescribing processes Experience using GP clinical systems i.e. EMIS Audit or service improvement experience

Qualifications Essential

Registered Pharmacist with the GPhC Strong knowledge of medicines management in primary care

Desirable

Independent Prescriber status Or willingness to undertake training if not yet qualified
