Job summary

We are part of Suffolk Primary Care, which is a group of GP practices delivering patient care across Suffolk. We are seeking to recruit an enthusiastic and flexible Clinical Pharmacist, with excellent interpersonal skills, to join our fantastic team at Framlingham Medical Practicein Framlingham, Suffolk.

The vacancy is for between 25-30 hours a week; however, this may be negotiable for the right candidate. Flexibility is crucial to accommodate training, holidays and occasional sickness.

Framlingham Medical Practicesupports in excess of 9,000patients from a diverse range of backgrounds. We have an established team with a strong focus on customer service and on developing and nurturing staff in order to deliver excellent standards of patient care. We would love to hear from candidates with healthcare experience. We are also keen to hear from those that can show flexibility and a range of transferable skills.

We use SystmOne and Anima to facilitate effective and efficient ways of working.

You should be able to work calmly and professionally as part of a team, with the ability to multi-task. A strong command of written and spoken English with an excellent telephone manner is a must. Good time management skills and the ability to follow protocols and or instruction and attention to detail is essential.

Main duties of the job

To reduce the medicines related workload on doctorsIncrease safety and quality of prescribed medicinesProvide support on medicines related issues to the whole practice teamBe a point of contact for patients regarding medicines queriesClinical Medication ReviewChronic Disease Management (for example, managing hypertension/Type 2Diabetes/Asthma/COPD patients, polypharmacy review)Medicines reconciliation on transfer between care providersCare home/Frailty medication reviewFocusing on QOF Domains, improving disease registersPrescribing Safety audits, Clinical audit, Local Enhanced Services supportLead on prescription management (systems and processes, ensuring monitoring is being done, support clerical staff)Actioning of MHRA safety alertsTo develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.To plan and organise own workload, including audit and project work, and training sessions for members of the practice team, patients, and carers.To maintain registration as a pharmacist and comply with appropriate professional codes.As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments,To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager.

Job description Job responsibilities

The post holder is a Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice within the wider PCN.

They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.

The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. This includes being a conduit of patient medicines information into and out of hospital. They will work to optimise medication issues to improve patient care and safety, and support clinical staff in the management of patients.

As part of their employment, the post holder will achieve qualifications from an accredited training pathway, equipping the Clinical Pharmacist to be able to practise safely and effectively in a primary care setting.

The supervision and mentorship network will be provided to support the Clinical Pharmacist.

Person Specification

Qualifications Essential

GPhC registered Pharmacist Hold or be working towards the GPhC independent prescribing qualification

Desirable

Membership of the Royal Pharmaceutical Society Minor ailments certification Medicines management qualification

Experience Essential

An appreciation of the nature of GP's and General Practice. An appreciation of the nature of primary care prescribing. Experience and awareness of common acute and chronic conditions that are likely to be seen in general practice. An appreciation of the new NHS landscape, including relationships between individual practices, PCNs and the commissioners.

Desirable

Minimum of 2 years working as a pharmacist demonstrated within a practice portfolio Experience in managing pharmacy services in primary care Un-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Understanding of the mentorship process Broad knowledge of general practice
