Job summary
Farnham PCN has an exciting opportunity for a further Clinical Pharmacist.
We are seeking an enthusiastic and forward-thinking pharmacist to work alongside our PCN Pharmacy Team.
This is an excellent opportunity to become an integral part of the primary care team.
Main duties of the job
The role will include delivering effective medicines management and medicines optimisation, improving patient care and being part of a multidisciplinary team whilst working within the strategic objectives of the PCN.

Job description Job responsibilities
For this role, primary care experience and completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP) would be an advantage however is not essential.
If you have not yet enrolled or completed, you should be willing to enrol on the CPPE Primary Care Pharmacy Education Pathway.
Excellent communication and interpersonal skills are essential plus being able to work as part of the practice team as well as autonomously.

Person Specification
Experience Essential
Experience working in community, primary care or clinical environment within a hospital. 2 or more years post registration experience.
Desirable
Clear evidence of further personal development. Previous working within GP practices. Knowledge of current NHS policy. Experience of multidisciplinary working.
Qualifications Essential
Masters degree in pharmacy (MPharm) (or equivalent) Registered with the General Pharmaceutical Council
Desirable
Postgraduate Qualification such as clinical diploma or demonstrable experience of advanced theoretical and practical knowledge GPhC Independent Prescribing Qualification
