Job Description Summary We are seeking a Home Health Referral Coordinator to join our Referrals Team. In this role, you will manage referral workflows, ensuring timely approvals, accurate documentation, and seamless patient care transitions to Home Health Agencies. You will apply knowledge of payer requirements and clinical systems to support operational processes, resolve routine issues, and collaborate with providers, payers, and internal teams to ensure efficient patient access to care.

• Process and track referral requests from intake through resolution, verifying payer requirements, clinical documentation, and coverage for compliance. • Manage moderately non-routine situations — including complex authorizations, urgent requests, and appeals — applying judgment within established payer and organizational guidelines. • Communicate directly with patients, providers, and payers to obtain missing information, relay authorization decisions, and resolve routine discrepancies. • Maintain accurate authorization and referral records in the EHR and authorization management platforms, ensuring data integrity and timely status updates. • Identify recurring processing challenges and communicate patterns to supervisors to support workflow improvements. • Serve as a liaison for patients requiring Home Health services by assessing their needs, matching them with appropriate home health agencies, and guiding them through the referral process until their first home health visit is successfully completed.
What We're Looking For
• High school diploma required; associate degree preferred. • 1–3 years of prior authorization, referral coordination, or managed care experience. • Bilingual in English and Spanish • Working knowledge of insurance products, payer authorization requirements, billing, and EHR systems (Athena - preferred). • Ability to manage multiple open cases simultaneously with accuracy under moderate supervision. • Strong written and verbal communication skills for patient, provider, and payer interactions.
