Job summary
Kent and Medway Mental Health NHS Trust is seeking a dedicated Registered Mental Health Nurse, Occupational Therapist, Social Worker, or qualified CYP-IAPT practitioner to join our Canterbury CYPMHS team as a Multidisciplinary Pathway Worker.
In this role, you will deliver high-quality clinical assessments, formulation, care planning, and evidence-based interventions for children and young people up to age 18 who present with emotional, behavioural, neurodevelopmental, and mental health needs.
You will manage an allocated caseload within structured CYPMHS pathways, combining autonomous clinical practice with multi-agency working alongside education, social care, primary care, and voluntary-sector partners. Supported by regular clinical supervision and a welcoming multidisciplinary team, you will contribute directly to high standards of care, safeguarding, and service improvement.
Main duties of the job
Clinical Assessment & Formulation: Undertake comprehensive assessments covering mental state, risk, safeguarding, and psychosocial needs to co-produce robust care plans and risk-management strategies with children, young people, and their families.
Evidence-Based Interventions: Manage an allocated caseload and deliver child-centred, family-focused interventions aligned with NICE guidelines and CYP-IAPT principles (such as CBT, Systemic Family Practice, psychoeducation, and parent support).
Outcome Monitoring: Use routine outcome measures to track therapeutic progress, review care plans, and ensure positive outcomes for service users.
Multi-Agency Collaboration: Partner effectively with local schools, GPs, paediatric teams, social services, and third-sector organisations to deliver integrated support.
Governance & Service Improvement: Participate actively in MDT meetings, clinical reviews, audits, safeguarding discussions, and service development initiatives.
Team Support & Clinical Administration: Maintain accurate, timely electronic clinical records while supporting junior colleagues, trainees, or students and engaging in continuous professional development.

Person Specification
Training, Qualifications and Registration Essential
Successful completion of graduate training such as nursing, social work or occupational therapy, or equivalent substantial KMMH/CYPMHS clinical experience. Active registration with your relevant regulatory body (e.g., HCPC, NMC)
Desirable
Additional post-qualification training relevant to children and young people's mental health, risk assessment, neurodevelopmental needs, trauma informed practice or evidence-based psychological interventions.
Experience Essential
Experience of working with children and young people with emotional wellbeing and mental health difficulties, including complex, challenging or high-risk presentations. Experience of carrying out assessment and delivering clinical interventions within a multi-disciplinary setting, including work with parents/carers, schools and partner agencies.
Desirable
Experience of CYP-IAPT informed practice, school-based assessment, community based assessment, group work, parent/carer interventions, or supervising junior staff / mental health workers.
Knowledge and Skills Essential
Additional post-qualification training relevant to children and young people's mental health, risk assessment, neurodevelopmental needs, trauma informed practice or evidence-based psychological interventions. Knowledge of risk management and ability to monitor, assess and respond to risk to ensure safety. Knowledge of legislation in relation to children and young people, safeguarding procedures and policies. Ability to communicate clearly and effectively about complex and sensitive clinical matters, verbally and in writing
