Are you interested in being part of transforming and developing an innovative service? Humber's Learning Disability Intensive Support Team aims to support people with learning disabilities and their families/carers.
We are looking for a dynamic individual to support the multidisciplinary team and Learning Disability Service in supporting people under the team's care with physical health investigations, planning, and implementing interventions.
We are recruiting one full-time Band 4 Nursing Associate Practitioner. The role will involve working closely with nurses, care coordinators, psychologists, occupational therapists, and speech and language therapists, as well as the wider teams.
The Practitioner will focus on health action planning and liaising with primary care services to ensure that people with learning disabilities who are supported by the LD-IST receive a high standard of service.
The successful post holder will receive regular supervision and training to support them in carrying out their role successfully.
The successful candidate will work across the Intensive Support Team (IST) and the Learning Disability Inpatient Unit. This role provides the opportunity to develop skills and experience across two closely linked services while ensuring continuity of care for patients as they move through the Learning Disability Pathway. The role will also provide the opportunity to gain a wider understanding of the patient and family/carer journey.
Main duties of the job
To work under the direction of the Band 6 Specialist Nurses and Band 5 Registered Nurses to implement physical health interventions for service users with learning disabilities.
To liaise with primary healthcare services to implement reasonable adjustments for people with learning disabilities.
To demonstrate an understanding of learning disabilities and consider appropriate ways of working with patients, while advising colleagues on reasonable adjustments.
To communicate effectively and attend meetings where appropriate, working as part of the LD-IST multidisciplinary team. To support other members of the team in evaluating the service's effectiveness and completing audits when required. To document all work completed using the Trust's electronic record system in line with documentation standards and legislation. To attend relevant training to support the effective delivery of the role.

Qualifications, Education and Training Essential
GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent Range of work procedures and practices, majority non-routine; intermediate level theoretical knowledge. Diploma or equivalent appropriate qualification, e.g. foundation degree; or NVQ3 level qualification plus short courses or relevant experience to diploma level Healthcare qualification
Skills, Competencies and Personal Qualities Essential
Development of supportive relationships with clients, carers and other professionals Evidence of effective inter-personal communication skills Able to work on own initiative, following direction by the registered professional Ability to take part on reflective practice and clinical supervision activities
