Main duties of the job As a Social Worker at Elysium Healthcare, you'll be instrumental in transforming lives, particularly aiding the most vulnerable in achieving brighter futures. The role involves working within a skilled multidisciplinary team at The Woodmill in Cullumpton, focusing on teamwork to help patients transition through recovery. Responsibilities include managing a complex caseload, risk assessments, social care assessments, facilitating group activities, and contributing to Integrated Care Record meetings. Additional duties involve liaising with home authorities for service users during their admission and discharge process, and reporting to the Safeguarding Lead. With previous experience from the NHS or private care, you can join the multidisciplinary team at The Woodmill, Cullumpton which has a strong emphasis on teamwork, and where you will provide care for patients to step down through their recoverys.
As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future.
You report to the Safeguarding Lead, and will manage internal investigations and liaise with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can see your career grow and you achieve your career aspirations.

As a Social Worker you will be: Referrals to LADO and Local Authority in line with safeguarding and assessments Educating Hospital staff about relevant safeguarding matters. Completing a full annual social care reassessment within the CPA format. Facilitating access to welfare rights, housing, advocacy services and associated support services Participating in 1:1 supervision To be successful in this role, you'll need:

The Woodmill is a 18 bed unit creates a pathway for patients to step down through their recovery. Ivy is a 6 bed admission area and Rose is a 12 bed rehabilitation area, with the option to flex within the 12 bed area to a four bed section that can create a further step down with a separate entrance for those patients that are progressing well and are preparing for discharge to the community.
The aim of the service is to support women to improve their self-worth, build a better quality of life for themselves and to equip them with the skills needed for community living and increased independence.
