The Opportunity:
Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. P
