Job summary
The post holder will take a lead role in assessing an individuals need, navigating referrals and using professional judgement to navigate and coordinate the support needs. The role is pivotal in ensuring people are safely and appropriately transferred between care settings. Working within a multi-agency team, the postholder ensures timely and appropriate onward referrals to community health, social care, Primary care or voluntary sector services. This role involves multidisciplinary collaboration, and effective communication to avoid delays
Main duties of the job
Provide Coordination and navigation for the Local Navigation hub caseload across health and care services, alongside working closely with other Primary Care services.
Coordinate and manage the administrative functions of the Multi-disciplinary team meeting.
Take minutes of the MDT meetings and disseminate, chase progress against actions identified.
This is a pivitol role to the Local Navigation Hub and will be the interface between Primary Care, secondary Care, Community Care, Social Care, Mental Health, Out of Hours and Voluntary sector organisations.

Erewash Health Partnership (EHP) was established in 2018 and is a single GP Partnership . EHP employs approximately 250 people.
As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.
This role sits within the Team Up service which is a cross organisational team who provide support to patients who are housebound or have complex needs.
The Local Navigation hub is for health and care professionals to call when they need support or coordination of care.
This is supported by Community GPs who are the senior clinical descision makers.

Receive and collate information from health and social care colleagues including the voluntary sector plus out of hours calls to identify if further support is required.
Signpost or refer onward to relevant service as appropriate
Contribute to assessments to identify a specific need, to maintain independence in the place they call home (own home, residential home etc).
To ensure the IT requirements for recording activity are adhered to in collaboration with other team members
To support with the provision of agreed performance/activity data within the EHP data team.
Raise awareness of shared decision-making and decision support tools and assist people to be more prepared to have a shared decision making conversation.
Direct liaison with multi agencies to coordinate care for patients.
