Montgomery Medical Practice is seeking an enthusiastic, self-motivated and conscientious Nurse Practitioner, Practice Nurse, to join our team. You must have a flexible and adaptable approach to a varied and high volume workload which includes triage and face to face appointments. The ability to multi-task and to work under pressure is essential.
The successful applicant would be working alongside our current multidisciplinary team, and be responsible for the delivery of nursing services to our patient population. The role will include managing areas such as acute care, health promotion, chronic disease management and health prevention.
Main duties of the job
In consultation with patients and in line with current practice disease management protocols, develop care plans for health management, offering support and health education. Responsible for a number of clinical areas such immunisation clinics and well men and women health checks.
Making appropriate and timely referral of patients for further opinion and treatment.
You will be the first point of contact for patients presenting with undifferentiated, undiagnosed problems, using skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis management plan via triage and face to face appointments.
You will report to the Partners.
Training will be provided.
About us
Montgomery Medical Practice is a rural, dispensing GP Practice with around 8100 patients situated on the border of Montgomeryshire and Shropshire. Despite its idyllic rural location Montgomery is within a short 30 minute drive of Shrewsbury and an hour and a half from Birmingham. We have a multidisciplinary team that includes Physician Associates, Nurse Prescriber, Practice Nurses and Health Care Assistants.
We are a training practice. We use EMIS Web clinical system and Accurx.

The following are the core responsibilities of the Practice Nurse There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Prioritise health issues and intervene appropriately this includes nurse led triage and the assessment of patients with acute symptoms
To follow infection control procedures, and complete training as required.
Develop, implement, and embed health promotion and wellbeing programmes.
Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
Request and undertake the collection of pathological specimens. Process and interpret pathology and other test results as required, within scope.
Maintain chronic disease registers and provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary, specialist, care as required.
Maintain accurate clinical records in conjunction with extant legislation.
Be responsible for the implementation of an effective immunisation programme.
Support the team in dealing with clinical emergencies.

Knowledge and Skills Wound care/removal of sutures and staples. ECGs. Venepuncture. Chaperone procedure. Chronic Disease Management. Understanding of infection prevention and control measures. Knowledge of public health issues in the local area. Understanding the importance of evidence based practice. Broad knowledge of clinical governance. Ability to record accurate clinical notes. Evidence of continuing professional development CPD, commensurate with the role of a Practice Nurse. Access to own transport. Excellent communication skills written and oral. Strong IT skills. Effective time management Planning & Organising. Ability to work as a team member and autonomously. Ability to follow clinical policy and procedure.
Desirable
Requesting pathology tests and processing the results, advising patients accordingly. Immunisations, routine, childhood and GMS Travel.
