About this role

Job summary We are seeking an experienced and motivated Advanced Specialist Pharmacist to join our innovative Home Treatment Teams (HTTs). This exciting role provides expert clinical pharmacy leadership for people experiencing acute mental health crises who are receiving treatment in the community. Working as an embedded member of multidisciplinary teams across two HTTs, you will play a pivotal role in improving patient outcomes through medicines optimisation, independent prescribing, medication reviews, physical health monitoring, and specialist pharmaceutical care. There are 2 posts to cover our 4 Home Treatment Teams. Locations are outlined in the job description and site preferences can be discussed at interview.

Main duties of the job

The Advanced Specialist Pharmacist provides embedded, expert clinical pharmacy input within the Home Treatment Team (HTT), supporting safe and effective medicines management for patients experiencing acute mental health crises who are being treated in the community.

The post holder will lead on medicines reconciliation, support patient adherence, provide specialist input for high-risk medicines, and manage complex pharmaceutical care plans, working as an integral member of the multidisciplinary team to reduce medication-related harm and support safe transitions of care. The post-holder will work across two HTT's. The successful candidate will be an organised, flexible individual with excellent communication and interpersonal skills and an ability to work on their own initiative to:

Lead and deliver specialist clinical pharmacy services Practice as an independent prescriber or work towards this role Work collaboratively towards the goals and objectives of the HTTs and Pharmacy services.

Person Specification

Experience Essential

Significant post-registration experience of mental health, community and hospital pharmacy Medicines optimisation in mental health services Managing patients prescribed high-risk medicines including clozapine and lithium Multidisciplinary mental health team working and making clinically appropriate interventions Audit, quality improvement and service development Leadership, supervision or professional support of other staff Provision of information to patients and carers Experienced in conducting medication reviews, making clinical prescribing recommendations and/or independent prescribing.

Desirable

Working within a Home Treatment Team, crisis, or community mental health setting

Qualifications and Professional Registration Essential

Registered with General Pharmaceutical Council Degree in Pharmacy (MPharm or equivalent qualification) Postgraduate Diploma in Clinical Pharmacy (or equivalent experience) Postgraduate Certificate in Psychiatric Therapeutics (essential or should be willing to complete within 2 years of joining Trust or equivalent qualification) Independent pharmacist prescriber (essential or willingness to obtain within an agreed timeframe)

Member of Royal College of Pharmacy Member of College of Mental Health Pharmacy
