Job summary St Peters Hospice are looking to recruit additional Palliative Care Support Workers to our Hospice at Home team providing care in the community across Bristol, North Somerset and South Gloucestershire.
If you are a dedicated and experienced healthcare assistant/support worker with an interest in working in end of life care, we would like to hear from you. The role of Senior Palliative Care Support Worker requires independent working in the community; applicants should have excellent communication and IT skills. You must be adaptable, reliable and highly organised to prioritise your workload effectively. Ideally you will have experience of working with people approaching the end of their life, along with some previous experience of working in the community.
This is a role where no two days are the same and you have the chance to improve the experience of patients and families when life is at its most difficult for them.
Main duties of the job This is a Band 3 level role; however, we do have development opportunities available in this team which would be offered on Band 2 whilst training and support is provided to reach Band 3. This can be discussed at interview if applicable.
You must have a clean driving licence, relevant insurance and access to a car for all shifts.

Job description Job responsibilities To provide holistic palliative care in the last 4 weeks of life To provide emotional, physical and psychological support to patients, their families and carers To work as part of the Hospice at Home team providing packages of care and support for patients in their own homes. Colleagues usually work in pairs but there is some lone working required. Day shift times 0745 2000, 0800-1600, 1200-2000. All colleagues are required to work a variety of shifts across 7 days as part of an 8 week rolling rota. Occasional nights may be required but are not a significant part of current advertised role. Night shift times 2115-0800. To work autonomously to provide planned care for individual patients with the support of, and accountability too, the Hospice at Home Registered nurses To role model the values of the hospice at all times by following the St Peters Hospice PCSW Code of Conduct and to contribute to a positive team culture To promote the safety and well-being of patients and their families at the end of life by following Hospice policies and procedures and working as part of a wider multi-disciplinary team.
Person Specification
Qualifications Essential
A nationally recognised qualification/apprenticeship in health & social care at Level 2 or above or the equivalent level of knowledge and skills gained through relevant experience
Desirable
A national recognised qualification/apprenticeship in health & social care, or palliative care, at Level 3 or above (e.g., City & Guilds Level 3 Certificate in End of Life Care)
