Job summary
Sudbury & West Suffolk Rural (S&WSR) Primary Care Network hasanexcitingopportunity for a social prescriber to join our practice teamon afull-time basis for 16 hours per week.
Social prescribers are responsible for providing non-clinical support by linking patients to community and voluntary services across their area, adopting a holistic approach to supporting patients to improve their health and wellbeing.We are looking for someone to join our team in order to provide this service across S&WSR PCN at, Clare Guildhall, and Siam Sudbury Surgeries.
Main duties of the job
The successful candidate will be responsible for providing one-to-one support, identifying issues that impact on patients' health and wellbeing, developing personalised care and supporting plans to address their needs, making onward referrals to community and voluntary sector services, and recording patient outcomes.

Person Specification
Qualifications Desirable
Ideally have Diploma Level 2/3 in Health and Social Care, psychology, sociology or public health. or equivalent qualification or experience;
Experience Essential
Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups; Strong listening and communication skills; You will be familiar with local resources and services and how to access them; Motivated by helping people, with care and empathy; Ability to deliver person-centred support in a non-judgmental way; Experience of completing holistic, person-centred care-planning assessments; Understanding of the needs of small, volunteer-led community groups and the ability to support their development; Commitment to collaborative working with all local agencies; Experience and confidence in the delivery of lifestyle change interventions.
