About this role

Job summary

Jorvik Gillygate Practice is looking for enthusiastic, motivated and compassionate Care Navigators to join our established team.

This role was previously known as a GP Receptionist role, but has developed to include a wider Care Navigation and signposting function to help patients access the most appropriate care and support.

Our Care Navigators are often the first point of contact for patients and play an important role in helping them access the right care, advice or service. This is a varied and rewarding role for someone who enjoys working with people, can communicate confidently and sensitively, and works well as part of a busy team.

As a Care Navigator, you will provide a welcoming and professional first point of contact for patients, whether face-to-face, by telephone or through our digital systems.

You will help patients navigate the range of services available and ensure their enquiry is directed to the most appropriate member of the clinical or administrative team.

Previous experience in general practice or healthcare would be an advantage, but is not essential, as full training will be provided for the right candidate. We are also keen to support staff development and progression within the practice.

Applicants will need to be flexible and able to work across our three York sites when required.

Main duties of the job

Duties will include:

Greeting patients and visitors and providing a friendly and professional reception service

Answering telephone and face-to-face enquiries

Supporting patients to access care through the practices appointment and Total Triage systems

Care navigating and signposting patients to the most appropriate clinician, service or support

Booking and managing appointments

Registering new patients and ensuring information is accurately recorded

Receiving and accurately recording messages for clinical and administrative teams

Processing incoming correspondence and clinical documents

Scanning, filing and maintaining accurate electronic patient records

Processing referrals and supporting waiting-list administration

Supporting repeat prescription and medication administration processes where appropriate

Assisting with patient recalls and follow-up

Liaising with hospitals, community services and other healthcare providers

Managing clinical and administrative tasks and email enquiries

Chaperoning clinicians when required

Receiving and handling patient specimens in accordance with practice procedures

Maintaining patient confidentiality at all times

Providing cover across our three sites where required

Undertaking relevant training and keeping mandatory training up to date

The role is varied, and additional administrative duties may be undertaken as required to support the wider practice team.

Jorvik Gillygate Practice is a large, forward-thinking and well-established GP practice caring for a diverse patient population across three sites in York.

Our main practice is situated within Yorks historic city walls, with additional surgeries at South Bank and East Parade.

We have a large multidisciplinary team including GPs, nurses, advanced practitioners, pharmacists, First Contact Physiotherapists, mental health practitioners, social prescribers, care coordinators and an experienced administrative team.

Person Specification

Experience Essential

We are looking for someone who: Has excellent communication and customer-service skills Is friendly, approachable and able to remain calm under pressure Is confident communicating with people from a wide range of backgrounds Has good IT and administrative skills Is organised and able to prioritise competing demands Understands the importance of confidentiality Works well independently and as part of a team Is flexible and willing to work across our three practice sites Has a positive attitude and willingness to learn

Desirable

Previous experience within general practice, primary care or another healthcare setting Previous reception, customer-service or administrative experience Experience using clinical systems such as SystmOne. Experience of care navigation or signposting
