Licensed Practical Nurse (LPN)

SAW, Inc. is seeking a compassionate, dependable, and knowledgeable part-time Licensed Practical Nurse (LPN) to join our team in supporting adults with developmental disabilities. The LPN will work closely with the Nursing Supervisor and interdisciplinary team to promote the health, safety, and well-being of individuals in our Adult Day Program.

Key Responsibilities

• Assist with nursing assessments by collecting, documenting, and reporting objective and subjective health information.

• Provide nursing services and delegated duties within the scope of practice of an Ohio Licensed Practical Nurse.

• Participate in the development, implementation, and evaluation of individualized nursing care plans under the direction of an RN.

• Monitor individuals for changes in health status, health and safety concerns, and communicable or nuisance diseases.

• Communicate health concerns and significant findings promptly to the RN and other appropriate healthcare professionals.

• Maintain accurate, timely, and confidential health records and required documentation.

• Provide health education to individuals, families, guardians, caregivers, and staff in consultation with the RN.

• Administer medications and perform nursing procedures within the LPN scope of practice and applicable laws and regulations.

• Work collaboratively with the interdisciplinary team to support individuals' health and personal goals.

• Respond calmly and professionally to medical concerns and other challenging situations.

• Maintain confidentiality and comply with all agency policies, procedures, and applicable regulations.

• Report suspected or actual abuse, neglect, or other concerns as required.

• Perform other job-related duties as assigned.

What We're Looking For

Required Qualifications:

• Current Ohio Licensed Practical Nurse (LPN) license in good standing.

• Experience in public health, school nursing, clinical nursing, or working with individuals with developmental disabilities preferred.

• Valid Ohio driver's license and a satisfactory driving record.

• Current First Aid and CPR certification, or ability to obtain certification after hire.

• Ability to successfully complete all required state, agency, and position-specific training.

Knowledge & Skills

The successful candidate will have:

• Knowledge of LPN scope of practice and when to involve the RN.

• Understanding of developmental disabilities and related health and behavioral needs.

• Familiarity with conditions that may affect individuals with developmental disabilities, including seizures, cerebral palsy, sensory deficits, orthopedic conditions, and other physical disabilities.

• Knowledge of medications, particularly psychotropic and anticonvulsant medications, including their uses and potential side effects.

• Strong communication, documentation, and organizational skills.

• The ability to recognize and report changes in an individual's health status accurately and promptly.

• The ability to remain calm and professional when dealing with medical concerns or difficult situations.

• The ability to work effectively with individuals, families, guardians, caregivers, coworkers, healthcare professionals, and community partners.

• Must demonstrate reliable attendance and punctuality.

• Familiarity with technology and online learning platforms is required.

Details of Position

This is a part-time position that covers our Beachwood and Galaxy Center locations.

Hours of work: Monday - Friday, 20-29 hours/week based upon need.

$600 signing bonus

PTO and Holiday pay

Pay increase after 90 workdays
