Essential Responsibilities

• Conduct ongoing nursing assessments, screenings, and evaluations of individuals receiving services and develop individualized nursing care plans.

• Maintain an assigned nursing caseload and provide direct and delegated nursing supports.

• Evaluate nursing interventions, direct services, and specialized procedures that are not delegable.

• Provide consultation and nursing support on-site or through telecommunication.

• Initiate medical referrals and complete ancillary referrals when assigned.

• Provide supervision, guidance, and support to Licensed Practical Nurses (LPNs). Assign work responsibilities.

• Perform, train staff, and complete required documentation related to delegated nursing duties in accordance with Ohio Department of Developmental Disabilities (DODD), Ohio Board of Nursing (OBN) regulations, and SAW, Inc. policies and procedures.

• Serve as a resource to staff regarding nursing procedures and health-related concerns.

• Collaborate with individuals served, parents, guardians, caregivers, physicians, dentists, community agencies, and other healthcare professionals.

• Participate in interdisciplinary assessments, referrals, problem-solving activities, and case conferences as needed.

• Ensure health records, delegated nursing records, logs, and required forms are complete and compliant with applicable regulations.

Qualifications

• Current Ohio Registered Nurse (RN) license in good standing with at least 18 months full-time experience.

• Prior experience working with individuals with developmental disabilities.

• Successful completion of DODD's RN Train-the-Trainer program (may be acquired post-hire).

• Certification to instruct CPR/First Aid/AED (may be acquired post-hire).

• Strong assessment, documentation, and communication skills.

• Ability to work independently and collaboratively within an interdisciplinary team.

• Valid driver's license and reliable transportation to travel between program sites.

• Must be able to pass drug test and background screening.

Work Schedule:

Monday - Friday. 20 hours per week -- days and hours dependent upon program needs (between

7:15 a.m. - 4:15 p.m.)
