Job summary
Older Persons Mental Health Team in Portsmouth are looking for a motivated, enthusiastic practitioner to join our friendly team, who provide skilled assessments and treatment interventions to the older adult.
To be a successful applicant you will need to be a registered nurse, preferably RMN, with experience in the care of older adult with mental health problems. You must be enthusiastic inworking with older adults to provide compassionate care to patients and their families and have knowledge of dementia and functional illness with a good understanding of physical health and frailty.
You will be required to work within the wider MDT team, implementing and evaluating care, managing your caseload, supervising HCSWs, junior staff and students within the team.
You will play an important role in our team, as we aspire to provide excellent care to our patients and strive to improve and develop our service in line with national and local guidance.
Access to a car for this role and a full driving licence is essential.

Main duties of the job
The Postholder will work within the Older Persons Community Mental health Team providing high quality mental health nursing care to adults over the age of 65 with complex mental health psychiatric conditions / dementia and those under 65 with dementia.
To meet the needs of patients and their families to achieve improved mental health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.
To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better. This is in line with the requirements set out in the NMC code which require all registered nurses to Prioritise people; Practice effectively; Preserve safety; Promote professionalism and trust.

Qualifications Essential
BSc Mental Health Nursing or Registered Mental Health Nurse currently registered with NMC. Practice assessor/supervisor. Post registration, specialist training relevant to specialty. Access to a car and a full driving licence

Experience Essential
Experience working and supporting patients and families, managing difficult and challenging mental health conditions including behavioural and psychological side effects of dementia, and patients who are nearing end of life. Experienced in mental health assessment. Experienced in developing care plans, delivering care and clinical interventions. Experience of Multi-Disciplinary Team working. Evidence of recent CPD. Evidence of developing policy, guidelines contributing to management of resources. Ability to prioritise workload and work autonomously, instruct and direct others; work is managed, not supervised. Experience in dealing with managing emotional issues. Clinically competent at Band 6 level. Experience of teaching, training, supervising and assessing skills. Treat patients with dignity, compassion and respect always. Be non-judgemental and inclusive, respecting diversity in background, culture and beliefs. Working with patients in their own homes and care / nursing home environment. Excellent communication skills, written and verbal. Ability to receive and provide complex information which may be sensitive / contentious, where negotiation may be required and there could be barriers to understanding.
