About this role

Job summary

The Community Rehab Service (CRS) Team is seeking an enthusiastic and experienced Lead Care Co-ordinator to support the delivery of high-quality, person-centred care across the community.

Working closely with the Team leads, and multidisciplinary teams, you will play a key role in ensuring safe and effective service delivery. You will lead the coordination of care, oversee staffing deployment, manage rosters, support recruitment and induction, undertake supervision and appraisal processes, and contribute to quality improvement initiatives.

This is an exciting opportunity for someone with strong organisational skills, leadership experience and a passion for improving outcomes for patients. The role involves close collaboration with healthcare professionals, patients, families and partner organisations to ensure care is delivered safely, efficiently and in line with service standards.

Main duties of the job

*Provide person centred, outcome focussed, support plans and risk assessments for the people we support; which are accessible and understood by all staff

*Encourage independence and self-management in a safe, secure home environment KM's and strive to restore self confidence in the individual being supported

*Ensure that therapeutic programmes of care as defined by clinical colleagues are understood and implemented as planned

*Organise and maintain satisfactory levels of support by deploying staff as necessary to meet the service needs over a 24-hour period, 7 days per week and 365 days per year

*Liaise with families, care managers, nursing, allied health professionals, the Inreach team and other providers to ensure the highest quality of support is available for each person we support.

*Maintained responsibility for the identification of continuing educational needs and development. Actively participate in the annual appraisal process and formulate a personal development plan

*Line manage the Care Co-ordinators, undertake annual appraisals, supervisions, manage absence and oversee their development

*Act as an assessor for staff undertaking NVQ levels 1 to 3

*To identify training needs of staff and empower staff to access training opportunities, whilst ensuring compliance with statutory and mandatory training

*To organise and lead, where required:
