We have an exciting opportunity for a Social Prescribing Link Worker to join our team! The role will be supporting four of our sites within the West Midlands Region: HMP Dovegate, HMP Stafford, HMP Featherstone & HMP YOI Drake Hall
Hours Monday - Friday: 8am - 4pm or 9am - 5pm
Salary This position is full time, youll receive an annual salary of £26,000 - £29,000 depending on experience.
Location You will manage up to 3 sites and spend one day a week at each with admin days at our West Midlands Regional Office. How will we support you?

Main duties of the job As a Social Prescribing Link Worker , your responsibilities will vary. You will
Complete initial assessments with patients referred to and eligible for the social prescribing service. Assessing their current wellbeing and identification of what matters to them. Manage a caseload of social prescribing patients providing follow-up support and ensuring they are able to access prescribed services with confidence. Be responsible for the scheduling and sequencing of patients social prescribing activities Liaise with Health and Prison departments, to develop awareness and support delivery of the service. Ensure effective cross-departmental working in order to meet our aims and objectives of a whole system and asset-based approach to social prescribing. Ensure services are consistent with our stated values.
