We have an exciting opportunity for a Transitional Worker Administrator to join team at HMP Brixton a very busy prison in London.
Hours
This is a full time role working Monday to Friday, 8.30am to 4.30pm.
Salary
Youll receive an annual salary from £27,000 to £33,000 per annum - depending on experience.

Main duties of the job
This is a patient facing role and as a Transitional Worker your responsibilities will vary. You will:
work collaboratively with the patient and other agencies to support a safe transition of care from custody to community manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment manage key risks associated with resettlement in the prison population provide support in the community, helping support their needs for hospital/ GP registrations/ appointments attend the complex case release meeting which is a multiple disciplinary meeting that includes representatives from safer custody, healthcare and external services If you are interested in career development, we have a wide range of opportunities for you to develop your skills and experience, which include both internal and external options for development and learning.

Qualifications, skills and experience required:
Experience of working in a prison, Mental Health or homelessness environment would be beneficial Administration experience Experience of working in multi-agency teams Experience of working with charities and similar organisations Knowledge of criminal justice systems in England Excellent communication skills and ability to communicate openly, consistently and clearly Proactive problem solving skills Secure environments are one of the most challenging, yet rewarding places for healthcare professionals to work.
