Salary: £36,540 - £40,610 per annum for 38 hours per week

(WTE £36,060 - £40,076 per annum for 37.5 hours per week)

Location: Based in Farnham with cross site working at the Beacon Centre, Guildford

We're looking for a registered nurse with expertise in palliative and end of life care, to join our community team, to provide specialist palliative care to patients and their families and carers in the community.

We're looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. Youll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the communityproviding holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care. We are excited to offer this opportunity as current post holders have been successful in progressing into more senior roles within Phyllis Tuckwell.

About You:

A successful Registered Nurse will have: RGN Degree Level Qualification Current NMC registration in adult or general nursing Palliative / end of life care or long-term condition experience. At least one year of working as a registered nurse with demonstrable competencies related to end-of-life nursing care Car driver and owner Experience of multidisciplinary team working and with external partners Able to make autonomous decisions demonstrating decision-making, critical thinking and proactive caseload management

Post: Hospice at Home Registered Nurse

Accountable to: Team Lead, or designated other lead

Job Purpose and Summary:

To work as a proactive member of the Phyllis Tuckwell (PT) multidisciplinary community team. The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. This role will involve providing holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members. They will work alongside all community partners and with the Phyllis Tuckwell multi-professional team to ensure that the patients experience is seamless.

The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwells (PT) policies and procedures.

Opportunities exist to rotate from Hospice at Home onto the In-Patient Unit to further develop your knowledge and skills in palliative and end of life care and enhance awareness of the patient and family experience of end-of-life care.

Responsibilities

1.0 Clinical

1.1 To assess, plan, co-ordinate, implement and evaluate palliative care interventions based on an agreed plan of care that supports quality and timely care delivered to the patient and family or carer. Assessing the patients changing needs and liaising with other PT specialists or external partners.

1.2 To ensure that care is centred around the person, family and their carer and that effectiveness, clinical safety and a joint approach with community nursing colleagues underpins PT practice.

1.3 To work closely with PT colleagues, the District Nursing teams and other partners in the provision of personalised holistic care to patients, their families, and carers.

1.4 To ensure quality standards are maintained and constantly re-evaluated, in order to provide the highest standards of holistic care.

1.5 To ensure that clear and concise reports of clinical and other activity are maintained and monitors record keeping standards in accordance with NMC guidelines, community and PT policy.

1.6 To be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.

1.7 To act as a resource and support to palliative care colleagues through giving advice, information, support, and education.

1.8 To record clinical activity as required by the organisation and practice with due regard to confidentiality and data protection.

1.9 To report risks, accidents, incidents, negative feedback, and complaints to the Team Lead/ Manager same day.

1.10 To work with colleagues to assess their clinical competencies as appropriate, providing training and support.

2.0 Professional

2.1 To work in accordance with NMC Code and within the NMC standards.

2.2 To contribute to team cohesion, through ongoing supportive relationships and consideration of colleagues.

2.3 To make autonomous decisions that prioritise the patient's best interests whilst practicing safely within professional and personal limitations.

2.4 To identify and work to mitigate risks, and report accidents or incidents in a timely way, ensuring appropriate action and the optimum safety and wellbeing of patients, their families and colleagues.

2.5 To develop and maintain effective relationships with members of the Primary Health Care Team and all other agencies involved in delivering care in the community.

2.6 To understand and work within relevant legislation and PT philosophy, mission and values.

2.7 To liaise with professionalism and respect towards all members of the multidisciplinary team who contribute towards the Hospice at Home Team and the wider team supporting the patient and their family at home.

2.8 To represent Phyllis Tuckwell in the public arena and occasionally give educational talks ensuring that the reputation of PT is maintained and enhanced.

3.0 Managerial

3.1 To be familiar with guidelines and regulations relevant to the role and care setting, particularly those relating to clinical and information governance, health and safety at work, and ensure they are met

3.2 To adhere to all Phyllis Tuckwell Policies and Procedures and promote this with the wider team.

3.3 To be aware of the security of the Phyllis Tuckwell (PT) buildings in and out of office hours and report any concerns to the on-call Maintenance Team or on call manager.

3.4 To contribute towards ensuring that exposure to risk is minimised through safe practice and awareness of health and safety and infection prevention and control.

3.5 To report any feedback, concerns and complaints from patients, their relatives, or others acting on their behalf, to the line manager.

3.6 To report any incidents or accidents, managing these via the reporting system. Escalating these as appropriate.

3.7 To ensure communication between members of the Hospice at Home team is effective so the care needs to patients are easily transferred to the person or persons assuming continuity of care at shift changeovers.

4.0 Training and Development

a) Team

4.1 To support a team approach through being proactive and positive.

4.2 To act as an assessor and mentor to students who need to develop skills and acquire experience in the field of palliative care.

4.3 To support the professional development and safe practice of others.

B) Personal

4.4 To identify, develop and maintain own knowledge and skills through continual professional development, discussion with an assessor, and through PT Annual Appraisal Process.

4.5 To maintain up-to-date clinical experience and knowledge and be able to provide evidence of this for nursing revalidation. To strengthen and develop clinical skills across all care settings in conjunction with line manager.

5.0 Research

5.1 To participate in research and audit within PT participating where appropriate.

5.2 To implement findings of evidence-based practice.

6.0 Safeguarding, Equality, Diversity and Inclusion

Role holders are required to understand their responsibilities in the safeguarding context, ensuring that Phyllis Tuckwell reduces the risk of harm or abuse of adults and children at risk.

You are required to:

adhere to Phyllis Tuckwells Safeguarding Policy (held on the NEST), which supports the local authoritys multi agency safeguarding policy, the law (Care Act 2014) and Mental Capacity Act 2005 (including Deprivation of Liberty Safeguards).
