Job summary
An exciting opportunity has arisen for a Community Registered Nurse Associate to join our district nursing teams within the Chorley, South Ribble and Preston areas.
We are looking for a compassionate, motivated and dedicated registered nurse associate to join our friendly and supportive community nursing teams.
As a community registered nurse associate you will play a vital role in providing high quality, person-centred nursing care to patients in their own homes and community settings. You will work collaboratively with patients, families, carers and a range of health and social care professionals to promote independence, improve health outcomes and support people to remain safely in their own communities.
As a valued member of the team, you will work under the supervision of the DN team manager and DN team leader alongside registered nurses, registered nurse associates and community healthcare assistants.
Main duties of the job
o Provide safe, effective and evidence-based nursing care to patients in their own homes and community settings.o Undertake comprehensive assessments, develop care plans and regularly evaluate patients' needs.o Administer medications and treatments in accordance with clinical guidelines and individual care plans.o Provide wound care, continence care, catheter care and other clinical interventions as required.o Recognise and respond appropriately to changes in patients' health and wellbeing.o Support patients with long-term conditions, complex needs and end of life care.o Promote health, wellbeing, independence and self-management.o Maintain accurate and timely clinical records in accordance with professional and organisational standards.o Work effectively with GPs, community teams, hospitals, social care and other professionals to ensure coordinated care.o Provide support, advice and education to patients, families and carers.o Contribute to service development, clinical governance, audit and quality improvement initiatives.

Qualifications Essential
Nursing Associate Foundation Degree On-going registration with the NMC. Evidence of Continued Professional Development that supports clinical practice.
Knowledge Essential
Evidence of effective interpersonal skills Ability to problem solve and make clinical decisions Understanding of NMC standards and implications for practice
Skills Essential
Excellent communication, listening, decision-making and problem solving skills. Developed clinical skills with a clear ability to understand new concepts and ideas in clinical practice. Can provide evidence of recent continued professional development within the scope of nursing.
