We are currently recruiting for an Out of Hours Community Nursing Sister/Charge Nurse to join our busy yet rewarding team.
We are seeking to recruit someone with previous experience of working in a Community Nursing team who can provide leadership and support to develop staff in reaching their full potential.
You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community.
You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.
Main duties of the job
Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care in the Out Of Hours 19:00-23:00 Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies. Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients. Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers. Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice. A key member of the Community Team, providing robust clinical services for patients, maintaining positive communication links other members of the Multi-Disciplinary Team (Day DN-UCR-Westcall GP-Paramedics), providing high quality patient focused care using evidence-based practice. Participating in the delivery of highly skilled nursing care to patients, providing End of Life Care, wound and IV-line management and assisting patients to develop self-management skills. Your working hours would be mostly 19:00-23:00 We are keen to promote a genuine work-life balance - offering mobile working and flexible working arrangements.

Education, qualifications or training Essential
Registered Nurse NMC Registration Specialist knowledge acquired through post graduate short courses, training or equivalent experience, in a relevant field.

Previous Experience Essential
Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care. Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills. Knowledge, skills and abilities Essential
Proficient use of digital tools and applications including Word, Outlook and patient management systems.
Additional Requirements Essential
Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
