About this role

Job summary

The Enhanced Access Nurse will provide high-quality nursing care to patients accessing the PCN Enhanced Access Service. The post holder will be a skilled and versatile registered nurse with broad clinical experience, strong assessment and decision-making skills, and the ability to work autonomously within their scope of practice.

Main duties of the job

The Enhanced Access Service operates from 1 October 2026, Monday to Friday from 18:30 to 21.30 and Saturdays from 9.00 to 17:00. This is a sessional position offering flexible working arrangements, with post holders contracted to work a minimum number of hours and additional sessions available according to service demand and mutual agreement. The role is designed to support a flexible staffing model, with sessions potentially covered by more than one post holder to ensure service resilience and continuity of care.

The post holder will deliver planned and opportunistic care, support the management of long-term conditions, undertake routine nursing procedures, health promotion activities, and assist in improving access to primary care services outside core practice hours.

The post holder will work collaboratively with GPs, Phlebotomists, and the wider multidisciplinary team to ensure safe, effective and patient-centred care.

Job description Job responsibilities

Clinical Duties:

Deliver face-to-face nursing appointments within the Enhanced Access Service.Undertake health assessments and provide evidence-based nursing care.Perform cervical cytology screening (where qualified).Administer vaccinations and immunisations in accordance with national and local guidance.Carry out wound care, dressings, suture removal and other treatment room procedures.Monitor and support patients with long-term conditions such as diabetes, asthma, COPD and hypertension.Conduct NHS Health Checks and preventative health interventions.Provide lifestyle advice and health promotion to patients.Undertake routine observations and clinical monitoring.Recognise deteriorating patients and escalate concerns appropriately.Maintain accurate clinical records using EMIS Web and other clinical systems.Enhanced Access Service Responsibilities

Support delivery of the PCN's Enhanced Access service specification.Work across PCN sites as required.Ensure appointments are utilised effectively and efficiently.Promote continuity of care through effective communication with patients' registered practices.

Person Specification

Experience Essential

Experience working within General Practice, Community Nursing or Primary Care. Experience of delivering treatment room services. Experience of long-term condition management. Experience using electronic clinical records systems.

Desirable

General Practice Nurse experience. Experience working within a PCN or Enhanced Access service.

Skills and Knowledge Essential

Excellent clinical and communication skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of infection prevention and control. Understanding of safeguarding children and adults. Good organisational and time management skills.

Qualifications Essential

Registered Nurse with current NMC registration. Evidence of continuing professional development. Immunisation and vaccination training or willingness to undertake

Desirable

Cervical screening qualification. Chronic disease management qualification. Independent Prescriber qualification.
