About this role

Job summary

We know that service users recover quickly at home, and you will be an integral component for ensuring that this is done in a safe and specific way as an alternative to hospital admission, working alongside the wider MDT.

As a Band 6 Nurse in the CRHT, you will be responsible for providing urgent and emergency triage, advice and assessment for people in a mental health crisis. You will also be responsible for undertaking home treatment visits as indicated by the MDT. This will include the provision and delivery of highly skilled nursing care, including care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.

You will be required to work autonomously, understand capacity and flow of the service and the organisation, attend MDT meetings, undertake gatekeeping assessments for inpatient admissions and crisis house admissions, work collaborative with the crisis house to lead on care for admissions as a service and facilitate early discharge from the inpatient wards where clinically indicated. As a Band 6 Nurse in the CRHT, you will also provide clinical leadership and supervision for junior staff.

Main duties of the job

To provide a clinical service that is service user focussed, accessible and responsive on 24-hour, 7-day a week basis.

To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.

To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.

To recommend and refer, where treatment is indicated, to the most appropriate team/service.

To undertake gatekeeping assessments for inpatient admissions and crisis house admissions.

To work collaboratively with the crisis house to lead on care for admissions as a service.

To facilitate early discharge from the inpatient wards where clinically indicated.

To deliver home based treatment interventions for all clients, ensuring best practice is implemented.

Where appropriate to participate in Mental Health Act assessments so that alternatives to formal hospital admission are considered.

Person Specification

Experience Essential

Demonstrable post registration experience as a Registered Mental Health/Learning Disability Nurse, Social Worker or Occupational Therapist working with people who have complex mental health needs. Experience of Multi-Disciplinary Team (MDT) working. Skills in coordinating and planning inter agency approaches in mental health care Demonstrable experience of assessing high levels of risk and formulation of robust risk management plans including the ability to make decisions to ensure the safety of individuals and carers. Experience of conducting comprehensive and holistic mental health assessments which attend to needs across biopsychosocial domains. Experience in safeguarding (both adult and children) and specialist risk assessment. Supervising students, and junior members of staff when required. Experience of delivery evidence-based treatments and interventions. Co-producing care plans and crisis plans alongside client and families.

Desirable

Experience of working in a community setting. Previous experience of working in the environment where the post is held. Experience of working with statutory and non statutory agencies Involving service users and carers in care delivery Experience of working within a crisis service. Qualifications Essential

Registered Mental Health/Learning Disability Nurse, Social Worker or Occupational Therapist with current registration with relevant professional regulator. Evidence of Relevant Continuing Professional Development (CPD). Practice Educator training / experience of supporting students. Risk Assessment and Management Training.

Suicide Prevention Training. Supervision Training.

Knowledge Essential

Ability to use initiative and work without immediate supervision. Ability to demonstrate an understanding, empathic approach Skills in coordinating and planning interagency approaches in mental health care An understanding of current mental health issues. Specialist Knowledge and understanding of relevant legislation, including; Mental Capacity Act, Mental Health Act, Care Act, Human Rights Act

Desirable

Knowledge and understanding of the roles of other professionals working within mental health Excellent knowledge of community resources both statutory and non-statutory Detailed understanding of the principles of recovery Ability to assess and respond to individuals presenting with self-harming behaviours, implementing appropriate interventions including risk management, harm minimisation, and brief therapeutic strategies to ensure safety and promote recovery. Experience of working within trauma informed services

Skills Essential

Ability to work independently and exercise professional autonomy, while recognising risk and escalating concerns appropriately. Experience in developing and implementing Risk Management Plans in collaboration with multidisciplinary teams and partner agencies. Demonstrable skills in clinical risk assessment, contingency planning, and managing individuals in acute distress Competent in delivering brief psychological interventions, including crisis management and harm minimisation strategies. Knowledge and application of CBT-informed approaches in practice. Ability to manage high emotional distress and challenging behaviours, including verbal aggression. Experience supporting individuals with self harm behaviours, focusing on safety and recovery-oriented care.

Desirable

Excellent verbal and written communication skills with a clear and concise command of English Strong report-writing ability, ensuring accurate and professional documentation. Highly skilled in engagement and relationship building, including working with carers, families, and support networks. Strong organisational and time management skills, with the ability to prioritise workload dynamically in response to changing demands IT literate, with experience using Microsoft systems and willingness to learn new digital tools.
