Care co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. They can support people to become more active in their own health and care and are skilled in assessing peoples changing needs. Care co-ordinators are effective in bringing together multidisciplinary teams to support peoples complex health and care needs. The role aims to improve continuity of care, support the management of long-term conditions and frailty, reduce avoidable hospital admissions, and ensure residents receive coordinated and responsive healthcare services.
Main duties of the job
As part of the role, you will be providing dedicated admin support to our clinicians, dealing with telephone/email queries, scanning, postal duties, booking patients in/out of the internal system and making any follow up appointments as required. Coordinate care for residents across designated care homes registered with the practice. Act as a key contact for care home staff regarding non-clinical coordination of residents care. Support proactive care planning for residents, particularly those with complex health needs, frailty, dementia, or multiple long-term conditions. Coordinate and support regular care home MDT meetings involving GPs, pharmacists, district nurses, frailty teams, palliative care teams, community services, and social care professionals. Recognise and appropriately escalate safeguarding concerns affecting vulnerable adults. You will also be able to demonstrate excellent organisational skills and be able to communicate effectively and have previous experience in a busy office environment.

Duties and Responsibilities as Care Coordinator
Providing dedicated admin support to our clinicians, dealing with telephone/email queries, scanning, postal duties, booking patients in/out of the internal system and making any follow up appointments as required.
Coordinate care for residents across designated care homes registered with the practice.
Act as a key contact for care home staff regarding non-clinical coordination of residents care.

Create and achieve a balance in the workload by prioritising work as necessary and monitoring the situation on a daily basis.
Maintain information systems including EMIS, Ardens Manager and Docman in accordance with directives from the Clinical Leads/Practice Manager.
To support the booking system, ensuring all appointments and client information is recorded on EMIS.
Build effective working relationships with care home managers and staff
Liaise with residents, relatives, carers, and advocacy services where appropriate.
Provide clear communication and support to ensure residents and families understand available services and care pathways.
