About the Opportunity We're seeking an experienced Care Coordinator (Registered Nurse) to join our growing National Care Coordination Centre (CCC) team. This is a Permanent Full-Time position based in Brisbane, Queensland, offering an exciting opportunity to be part of a nationally connected service transforming how healthcare is delivered across Australia. As a Care Coordinator, you'll play a pivotal role in managing the patient journey from referral and admission through to discharge and transition of care. Working within a multidisciplinary team, you'll coordinate services, assess clinical needs, manage patient flow and support individuals to safely navigate their care journey through innovative virtual and home-based care pathways. This is an ideal opportunity for a nurse who is passionate about care coordination, enjoys working across innovative models of care and wants to contribute to one of Australia's most exciting healthcare transformations.

Key Responsibilities • Conduct referral triage, clinical intake assessments and admission processes. • Coordinate and monitor patient journeys across virtual and home-based care programs. • Provide clinical support, education and guidance to patients, families and carers. • Liaise with referrers, treating clinicians, external service providers and multidisciplinary teams. • Identify, monitor and escalate clinical risks, deterioration and urgent care needs. • Support discharge planning, care transitions and continuity of care activities. • Coordinate treatment pathways, including equipment, consumables and service requirements. • Utilise digital care platforms to support care coordination, monitoring, scheduling and escalation pathways. • Facilitate virtual ward rounds, complex case reviews and multidisciplinary care discussions. • Manage a shared caseload while contributing to workload balancing and patient flow across the service. • Maintain accurate clinical documentation, patient records and activity reporting. • Contribute to quality improvement, service development and innovation initiatives. • Support the implementation and growth of contemporary, technology-enabled models of care.

About You You're a compassionate and experienced Registered Nurse who thrives in collaborative environments and is passionate about delivering outstanding patient outcomes.You bring strong clinical judgement, excellent communication skills and a desire to help shape the future of connected healthcare through innovative models of care.

You will bring: • Relevant tertiary qualifications in Nursing. • Current unrestricted registration with AHPRA as a Registered Nurse. • Minimum five (5) years' clinical experience within acute, community, subacute, chronic disease management or care coordination settings. • Comprehensive assessment, clinical management and care coordination skills. • Experience with discharge planning and working collaboratively with internal and external healthcare providers. • Strong communication and relationship management skills. • Ability to work effectively within multidisciplinary and geographically dispersed teams. • Excellent organisational, time management and prioritisation skills. • Confidence working within digital and technology-enabled models of care, including telephone and video-based care delivery. • Strong digital literacy and experience using clinical information systems and electronic health records. • A flexible, positive and collaborative approach with a growth mindset. • Commitment to continuous improvement, patient safety and evidence-based practice.

Highly Regarded • Postgraduate qualifications in a relevant clinical specialty. • Experience within virtual care, telehealth, Hospital in the Home, community nursing or care coordination programs. • Knowledge of the private health insurance landscape and healthcare funding models. • Experience using digital care platforms or remote patient monitoring technologies.
